全腹腔镜远端胃癌根治术治疗老年胃癌患者的疗效评价

摘　　要：目的:探讨全腹腔镜远端胃癌根治术治疗老年胃癌患者的临床疗效。方法:选择2019年1月~2023年12月本院收治确诊为胃癌的90例老年患者为研究对象,根据患者手术方式不同将其分为两组,对照组(45例)实施腹腔镜远端胃癌根治术,观察组(45例)实施全腹腔镜远端胃癌根治术,对两组术后效果进行比较。结果:术前,两组患者淋巴结数量对比,未见显著差异(P>0.05),而术中观察组患者术中出血量、手术切口长度、手术时间及平均住院日均较对照组更少(短)(P<0.05)。术前,观察组及对照组患者前列腺素E2、促肾上腺皮质释放激素、促肾上腺皮质激素创伤反应指标对比无显著差异(P>0.05),术后24h观察组患者前列腺素E2、促肾上腺皮质释放激素、促肾上腺皮质激素指标均显著低于对照组(P<0.05)。观察组并发症发生率明显低于对照组(P<0.05)。结论:相较于腹腔镜辅助远端胃癌根治术,全腹腔镜远端胃癌根治术具有创伤小、术后并发症风险低、术中时间及平均住院日更短的优势。Objective:To explore the clinical efficacy of total laparoscopic distal gastrectomy for elderly patients with gastric cancer.Methods:90 elderly patients diagnosed with gastric cancer admitted to our hospital from January 2019 to December 2023 were selected as the study subjects.They were divided into two groups based on different surgical methods.The control group(45 cases)received laparoscopic distal gastrectomy(LADG),while the observation group(45 cases)received total laparoscopic distal gastrectomy(TLDG).The postoperative effects of the two groups were compared.Results:Before surgery,there was no significant difference in the number of lymph nodes between the two groups of patients(P>0.05),while the intraoperative observation group had fewer(shorter)intraoperative bleeding,surgical incision length,surgical time,and average hospital stay compared to the control group(P<0.05).Before surgery,there was no significant difference in the PGE2,CRH,and ACTH trauma response indicators between the observation group and the control group(P>0.05).However,24 hours after surgery,the PGE2,CRH,and ACTH indicators in the observation group were significantly lower than those in the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:Compared with laparoscopic assisted distal gastric cancer radical surgery,total laparoscopic distal gastric cancer radical surgery has the advantages of less trauma,lower risk of postoperative complications,shorter intraoperative time and average hospital stay.
