摘　　要：目的探讨血清中白细胞介素33(IL-33)在中重度斑块型银屑病患者中的表达及其与颈动脉粥样硬化的关系。方法纳入2023年1月—2023年12月新乡医学院第一附属医院皮肤科160例中重度斑块型银屑病患者(银屑病组)和60例健康体检者(对照组)作为研究对象,收集所有中重度斑块型银屑病患者的临床资料。采用ELISA法检测血清中IL-33表达水平。对所有中重度斑块型银屑病患者颈动脉内膜中层厚度(intima-media thickness,IMT)进行检测,并分为颈动脉粥样硬化斑块组(n=52)和非颈动脉粥样硬化斑块组(n=108)。采用Pearson法分析IL-33表达水平与颈动脉IMT的相关性,Logistic回归分析患者颈动脉粥样硬化斑块形成的危险因素,采用ROC曲线分析患者血清中IL-33在诊断颈动脉粥样硬化斑块形成的临床价值。结果银屑病组和对照组血清中IL-33表达水平分别为(30.54±2.91)pg/mL、(17.34±1.67)pg/mL,血清中IL-33表达水平在银屑病组中显著升高,差异有统计学意义(t=33.000,P<0.001)。颈动脉粥样硬化斑块组52例中重度银屑病患者血清中IL-33表达水平为(36.01±3.76)pg/mL,颈动脉IMT平均为(1.62±0.14)mm,血清中IL-33表达水平与颈动脉IMT具有显著正相关性,相关系数r=0.784,P=0.012。性别、年龄、体重、病程、吸烟饮酒史、高血压病史、糖尿病病史、收缩压、舒张压、血糖、高密度脂蛋白胆固醇(HDL-c)、尿酸、谷草转氨酶(AST)、谷丙转氨酶(ALT)、肌酐及血沉(ESR)在非颈动脉粥样硬化斑块组和颈动脉粥样硬化斑块组中相比差异无统计学意义(P>0.05);与非颈动脉粥样硬化斑块组相比,BMI、PASI评分、低密度脂蛋白胆固醇(LDL-c)、甘油三酯(TG)、总胆固醇(TC)、超敏C反应蛋白(hs-CRP)、白细胞(WBC)、中性粒细胞(N)及IL-33表达水平在颈动脉粥样硬化斑块组中显著升高,差异有统计学意义(P<0.05)。Logistic回归分析结果显示,LDL-c升高(OR=3.642)和IL-33升高(OR=4Objective To investigate the serum expression level of interleukin-33(IL-33)in patients with moderate to severe plaque psoriasis and its relationship with carotid atherosclerosis.Methods A total of 160 patients with moderate to severe plaque psoriasis(psoriasis group)and 60 healthy controls(control group)from the First Affiliated Hospital of Xinxiang Medical University between January and December 2023 were enrolled.The clinical data of all patients with moderate to severe plaque psoriasis were collected.The serum expression level of IL-33 was detected by ELISA methods.All patients with moderate to severe plaque psoriasis completed with intima-media thickness(IMT)detection and further divided into carotid atherosclerotic plaque group(n=52)and non-carotid atherosclerotic plaque group(n=108),respectively.The correlation between IL-33 expression level and carotid IMT was analyzed by Pearson method.The risk factors of carotid atherosclerotic plaque formation in patients with moderate to severe plaque psoriasis were analyzed by Logistic regression.The clinical value of serum IL-33 levels in the diagnosis of carotid atherosclerotic plaque formation in patients with moderate to severe plaque psoriasis was analyzed by ROC curve.Results The serum IL-33 expression level in psoriasis group and control group was(30.54±2.91)pg/mL and(17.34±1.67)pg/mL,respectively,and the serum IL-33 expression level in psoriasis group was significantly increased,with statistical significance(t=33.000,P<0.001).The serum expression level of IL-33 in 52 patients with moderate to severe psoriasis from carotid atherosclerotic plaque group was(36.01±3.76)pg/mL,and the value of carotid IMT was(1.62±0.14)mm.There was a significant positive correlation between the serum expression level of IL-33 and carotid IMT(r=0.784,P=0.012).There were no significant differences in gender,age,weight,disease duration,smoking and drinking history,complicated with hypertension and diabetes,systolic blood pressure,diastolic blood pressure,blood glucose,HDL-c,uric a
