氟轻松氢醌维A酸乳膏治疗黄褐斑上市临床应用专家指导意见

摘　　要：作为国际上黄褐斑治疗一线药物,由氢醌、维A酸和糖皮质激素组成的三联乳膏(Kligman配方),因其独有的高效性和安全性,在国外被誉为黄褐斑局部治疗的金标准。我国黄褐斑诊疗专家共识中也提到,此三联配方局部联合使用可提高治疗效果。近期国内获批的氟轻松氢醌维A酸乳膏是我国首款上市治疗黄褐斑的“三联乳膏”,其含有0.01氟轻松、4氢醌和0.05维A酸3种活性成分,适用于在使用防晒剂等避光措施下,对面部中、重度黄褐斑的短期治疗。为了科学合理地使用氟轻松氢醌维A酸乳膏治疗黄褐斑,中国色素病学相关专家以氟轻松氢醌维A酸乳膏的药理作用机制、治疗黄褐斑的国内外临床研究结果、疗效和安全性研究、临床应用进展以及不良反应处理措施等为依据,讨论制定了本指导意见,供广大临床医生参考。As the first-line prescription cream topically used to treat melasma worldwide,the triple combination cream(TCC,Kligman formula)consisting of hydroquinone,tretinoin,and glucocorticoid has been well known as the gold standard for the topical treatment of melasma due to its satisfactory outcome and safety performance.It is also mentioned in China′s expert consensus on melasma treatment that the combined use of this triple formula can improve the therapeutic effect.Recently,the fluocinolone acetonide,hydroquinone and tretinoin cream is the first triple combination cream in China for the treatment of melasma,which contains 0.01 fluocinolone acetonide,4 hydroquinone and 0.05 tretinoin,and it is suitable for short-term treatment of moderate to severe facial melasma under the use of sunscreen and other sun avoidance procedures.To ensure the scientific and rational use of fluocinolone acetonide,hydroquinone and tretinoin cream in the treatment of melasma,Chinese pigment experts discussed and formulated the guideline for the reference of clinical dermatologists based on the pharmacological mechanisms of the cream,clinical research findings from both domestic and international studies on its efficacy and safety in treating melasma,advancements in clinical applications,and strategies for managing adverse reactions.
