摘　　要：目的探讨转化生长因子-β1(TGF-β1)、钙粒蛋白A(S100-A8)、白细胞介素-17(IL-17)对脊柱结核的诊断价值及与红细胞沉降率(ESR)、C反应蛋白(CRP)的关系。方法选取2017年7月至2023年12月于兰州大学第二医院确诊脊柱结核的103例患者为研究组,同期选取109例体检健康者为对照组。收集两组一般资料,比较两组TGF-β1、S100-A8、IL-17、ESR、CRP水平,运用受试者工作特征(ROC)曲线分析TGF-β1、S100-A8、IL-17对脊柱结核的诊断价值,运用Pearson相关分析研究脊柱结核患者TGF-β1、S100-A8、IL-17水平与ESR、CRP的相关性。结果研究组有高血压史、肺结核占比高于对照组(P<0.05),两组其余指标比较,差异无统计学意义(P>0.05);与对照组比较,研究组TGF-β1、S100-A8、IL-17、ESR、CRP水平升高(P<0.05);TGF-β1、S100-A8、IL-17水平与ESR、CRP均呈正相关(P<0.05);ROC曲线分析显示,TGF-β1、S100-A8、IL-17联合诊断脊柱结核的曲线下面积、灵敏度、特异度分别为0.907、97.1%、74.3%,诊断效能较高。结论TGF-β1、S100-A8、IL-17三者联合诊断效能更佳,且其与脊柱结核患者ESR、CRP均呈正相关。Objective To investigate the diagnostic value of transforming growth factor-β1(TGF-β1),calgranulin A8(S100-A8)and interleukin-17(IL-17)in spinal tuberculosis and their relationship with erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP).Methods A total of 103 patients diagnosed with spinal tuberculosis in the Second Hospital of Lanzhou University from July 2017 to December 2023 were selected as the study group,and 109 healthy people were selected as the control group during the same period.The general data of the two groups were collected,and the levels of TGF-β1,S100-A8,IL-17,ESR and CRP were compared between the two groups.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of TGF-β1,S100-A8 and IL-17 for spinal tuberculosis.Pearson correlation analysis was used to study the correlation between the levels of TGF-β1,S100-A8,IL-17 and ESR,CRP in patients with spinal tuberculosis.Results The proportion of hypertension and tuberculosis in the study group was higher than that in the control group(P<0.05),and there was no significant difference in other indicators between the two groups(P>0.05).Compared with the control group,the levels of TGF-β1,S100-A8,IL-17,ESR and CRP in the study group were increased(P<0.05).The levels of TGF-β1,S100-A8 and IL-17 were positively correlated with ESR and CRP(P<0.05).ROC curve analysis showed that the area under the curve,sensitivity and specificity of TGF-β1,S100-A8 and IL-17 in the diagnosis of spinal tuberculosis were 0.907,97.1%and 74.3%,respectively,and the diagnostic efficiency was high.Conclusion The combination of TGF-β1,S100-A8 and IL-17 has a better diagnostic efficacy,and it is positively correlated with ESR and CRP in patients with spinal tuberculosis.
