摘 要:目的探讨寻常型银屑病(PSO)患者病情程度与马拉色菌密度的关系,并分析P物质(SP)、肿瘤坏死因子-α(TNF-α)、皮损组织NLR家族含pyrin结构域蛋白3(NLRP3)炎症小体水平。方法选择2021年8月—2023年8月吉安市中心人民医院收治的PSO患者,根据马拉色菌感染情况分为感染组120例和未感染组130例。比较两组临床资料、血清SP、TNF-α及皮损组织NLRP3炎症小体及不同病情程度PSO患者马拉色菌密度、血清SP、TNF-α、NLRP3炎症小体。结果感染组重度病情患者占比42.50%,高于未感染组的32.31%,银屑病皮损面积和严重程度指数(PASI)为(16.65±3.65)分高于未感染组(P<0.001)。重度患者马拉色菌密度为“+++”的患者占比为66.67%,高于中度、轻度患者的40.91%、16.00%,中度患者高于轻度患者(P<0.05)。感染组血清SP、TNF-α及皮损组织NLRP3炎症小体表达水平分别为(94.76±15.54)ng/ml、(38.76±6.54)pg/ml、(1.87±0.99)均高于未感染组(P<0.05)。相较于中度、轻度患者,重度患者血清SP、TNF-α及皮损组织NLRP3炎症小体表达水平均更高,中度患者高于轻度患者(P<0.05)。结论马拉色菌感染可能会加重PSO患者病情,其密度与病情程度密切相关,且SP、TNF-α、NLRP3炎症小体可能与PSO的发生与发展具有密切关系。OBJECTIVE To investigate the relationship between the disease severity and density of Malassezia in patients with psoriasis vulgaris(PSO),and levels of substance P(SP),tumor necrosis factor-α(TNF-α),and NLR family containing pyrin domain protein 3(NLRP3)inflammasome in the PSO lesion tissue.METHODS Patients with PSO who admitted at Ji'an Central People's Hospital from Aug.2021 to Aug.2023 were selected and divided into 120 cases of the infected group and 130 cases of the uninfected group according to their Malassezia infection status.The clinical data,serum SP,TNF-αand NLRP3 inflammasome levels in lesions of two groups,as well as the density of Malassezia,serum SP,TNF-αand NLRP3 inflammasome levels in different severities of PSO were compared.RESULTS The proportion of severe patients ess in the infected group was 42.50%,higher than that of 32.31%in the uninfected group,and the psoriasis skin lesion area and severity index(PASI)was(16.65±3.65)points,higher than that in the uninfected group(P<o.oo1).The proportion of severe patients with Malassezia density of"+++"was 66.67%,higher than 40.91%and 16.00%of the moderate and the mild patients,and the proportion of the moderate patients was higher than that of the mild patients(P<0.05).The serum SP,TNF-αand NLRP3 inflammasomes expression levels in skin lesions in the infected group were(94.76±15.54)ng/ml,(38.76±6.54)pg/ml and(1.87±0.99),higher than those in the uninfected group(P<0.05).Compared with the moderate and mild patients,serum SP and TNF-α,NLRP3 inflammasomes expression levels in skin lesions in severe patients were higher,and in moderate patients were higher than those in mild patients(P<0.05).CONCLUSIONS Malassezia infection might aggravate the disease of patients with PSO,and its density is closely related to the severity of the disease,and the SP,TNF-α,NLRP3 inflammatory bodies might be closely related to the occurrence and development of PSO.
