摘  要:目的 探究肺炎支原体肺炎(MPP)感染患儿血清citron、载脂蛋白AⅠ(ApoAⅠ)、半胱氨酰白三烯(CysLTs)水平,并分析其对病情程度的预测价值。方法 选取苏州高新区人民医院2020年10月-2023年10月收治的MPP感染患儿112例作为病例组,同时选取同时期于医院健康体检的健康儿童118名作为对照组,根据病情将病例组分为轻度组(69例)和重度组(43例)亚组。比较两组、亚组血清citron、ApoAⅠ、CysLTs水平。采用Spearman分析citron、ApoAⅠ、CysLTs水平与MPP感染患儿病情程度的相关性,采用受试者工作特征(ROC)曲线分析citron、ApoAⅠ、CysLTs对MPP感染患儿诊断价值。结果 病例组citron、CysLTs分别为(4.32±1.42)ng/ml、(839.36±212.79)pg/ml高于对照组,病例组ApoAⅠ为(0.85±0.27)g/L低于对照组(P<0.05)。重度组citron、CysLTs高于轻度组,重度组ApoAⅠ低于轻度组(P<0.05)。citron、CysLTs水平与MPP感染患儿病情程度呈正相关(r=0.718、0.671,P均<0.05);ApoAⅠ则呈负相关(r=-0.749,P=0.009)。citron、ApoAⅠ、CysLTs联合检测诊断MPP感染的曲线下面积(AUC)高于单一检测(P<0.05),且联合检测的敏感度、特异度为89.29%、86.44%。结论 citron、ApoAⅠ、CysLTs在MPP感染患儿中表达异常,并与MPP感染患儿的病情程度有相关性,且三者联合检测可提高对MPP感染患儿的诊断价值。OBJECTIVE To investigate the expression of serum citron,apolipoprotein A I(ApoAI)and cysteine leukotriene(CysLTs)in children infected with Mycoplasma pneumoniae pneumonia(MPP)and their relationship with the disease severity.METHODS One hundred and twelve children with MPP infection admitted to Suzhou High tech Zone People's Hospital from Oct.2020 to Oct.2023 were selected as the case group,and 118 healthy children who underwent physical examination in the hospital during the same period were selected as the control group,and the case group were divided into mild(69 cases)and severe subgroups(43 cases)according to their disease conditions.The levels of serum citron,ApoA I and CysLTs were compared between the two groups and further subgroups,and the correlation between the levels of citron,ApoA I and CysLTs and the severity of MPP infection in children was analyzed by Spearman.The diagnostic value of citron,ApoA I and CysLTs for children with MPP infections was analyzed by receiver operating characteristic curve(ROC).RESULTS Citron and CysLTs in case group were(4.32±1.42)ng/ml and(839.36±212.79)pg/ml,respectively,which were higher than those in the control group,and ApoA I in the case group was(o.85±0.27)g/L,which was lower than that in the control group(P<o.05).Citron and CysLTs in the severe group were higher than those in the mild group,and ApoA I in the severe group were lower than that in the mild group(P<0.05).The levels of citron and CysLTs were positively correlated with the severity of illness in children with MPP infection(r=0.718,0.671,P<0.05),while ApoAI was negatively correlated(r=-0.749,P=0.009).The area under the curve(AUC)of citron,ApoAI and CysLTs combined for the diagnosis of MPP infections was higher than that of single detection(P<0.05),and the sensitivity and specificity of combined detection were 89.29%and 86.44%.CONCLUSIONS Citron,ApoAI and CysLTs are abnormally expressed in children with MPP infection and correlat with the severity of disease in children with MPP infection,and the combine
