摘  要:目的探讨支气管哮喘合并呼吸道感染患儿辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡、白细胞介素-6(IL-6)mRNA及信号转导及转录激活因子3(STAT 3)mRNA水平,以及各指标与感染的相关性。方法选择2022年9月—2023年9月青岛大学附属青岛市海慈医院收治的94例支气管哮喘合并呼吸道感染患儿(感染组)及71例单纯支气管哮喘患儿(未感染组)为研究对象进行回顾性分析,并选择同期于医院进行体检的健康儿童54名为健康组。比较三组Th17细胞、Treg细胞、IL-6、IL-6 mRNA及STAT3 mRNA水平,采用Pearson相关系数分析支气管哮喘合并呼吸道感染IL-6、IL-6 mRNA及STAT3 mRNA与Th17细胞、Treg细胞的相关性。结果感染组患儿的呼吸道病原体主要以呼吸道合胞病毒为主,占46.81%。相较于未感染组及健康组,感染组用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))及FEV_(1)/FVC分别为(2.31±0.36)L、(1.65±0.32)L及(0.74±0.13)L低于未感染组和健康组,且未感染组低于健康组(P<0.05);感染组Th17细胞、Th17/Treg、IL-6、IL-6mRNA及STAT3 mRNA水平分别为(2.58±0.63)%、(0.65±0.13)(16.24±4.45)ng/L、(6.81±1.92)及(1.79±0.43)高于未感染组和健康组,且未感染组高于健康组(P<0.05);感染组Treg细胞为(3.95±0.89)%低于未感染组和健康组,且未感染组低于健康组(P<0.05);Spearman相关性分析结果显示,IL-6 mRNA及STAT3 mRNA与Th17细胞均呈正相关(r=0.575,0.553,P均<0.001),与Treg细胞均呈负相关(r=-0.682,-0.533,P均<0.001)。结论呼吸道感染显著加重支气管哮喘患儿的Th17/Treg平衡失调,并激活IL-6/STAT3通路的活性。OBJECTIVE To investigate the balance of helper T cell 17(Th17)/regulatory T cell(Treg),interleukin-6(IL-6)mRNA and activator of transcription 3(STAT3)mRNA levels in children with bronchial asthma combined with respiratory infections,as well as the correlation between each index and infection.METHODS Ninety-four children with bronchial asthma complicated with respiratory tract infection(infection group)and 71 children with simple bronchial asthma(non-infection group)admitted to the Qingdao Hiser Hospital Affiliated of Qingdao University from Sep.2022 to Sep.2023 were selected for retrospectively analysis,and 54 healthy children who underwent physical examination in the hospital during the same period were selected as the control group.The levels of Th17 cells,Treg cells,IL-6,IL-6 mRNA and STAT3 mRNA were compared among the three groups,and Pearson correlation coeffient was used to analyze the correlation of IL-6,IL-6 mRNA and STAT3 mRNA with Th17 cells and Treg cells in children with bronchial asthma complicated with respiratory tract infection.RESULTS Respiratory pathogens in the infection group were mainly respiratory syncytial virus,accounting for 46.81%.The forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1))and FEV_(1)/FVC in the infection group were(2.31±0.36)L,(1.65±0.32)L and(0.74±0.13)L,respectively,which were low compared with those in the non-infection group and the control group,and the non-infection group was lower than the control group(P<0.05).The levels of Th17 cells,Th17/Treg,IL-6,IL-6 mRNA and STAT3 mRNA in the infectiongroupwere(2.58±0.63)%,(0.65±0.13),(16.24±4.45)ng/L,(6.81±1.92)and(1.79±0.43),respectively,which were higher than those in the non-infection group and the control group,and those in the non-infection group was higher than those in the control group(P<0.05).The Treg cells in the infection group was(3.95±0.89)%,lower than that in the non-infection group and the control group,and that in the non-infection group was lower than that in the control group(P<
