摘  要:目的探讨系统性红斑狼疮合并感染患儿辅助性T细胞(Th)22、Th17细胞及细胞因子和微小核糖核酸(miR)-200水平,并分析各指标对其预测价值。方法选取2020年3月—2023年4月中国医科大学附属盛京医院收治的150例系统性红斑狼疮(SLE)合并感染患儿为感染组,选取同期150例医院收治的SLE患儿为未感染组,另外选取150名健康体检儿童作为对照组。对感染组患儿进行为期一年的随访,根据有无器官损伤分为预后良好组和预后不良组,对比三组、预后良好组和预后不良组Th22、Th17细胞、白细胞介素(IL)-17、IL-22和miR-200相对表达水平,受试者工作特征(ROC)曲线分析Th22、Th17细胞、IL-17、IL-22和miR-200对系统性红斑狼疮合并感染患儿预后的预测价值。结果感染组miR-200为(1.14±0.27)低于未感染组(1.36±0.33)和对照组(P<0.05),感染组Th22、Th17细胞和IL-17、IL-22分别为(6.19±1.24)、(1.76±0.38)和(21.16±4.48)、(52.79±7.94)高于未感染组和对照组(P<0.05);预后不良组miR-200为(0.91±0.22)低于预后良好组(P<0.05),预后不良组Th22、Th17细胞和IL-17、IL-22分别为(6.86±1.35)、(1.97±0.36)和(24.36±5.03)、(58.51±7.45)高于预后良好组(P<0.05);Th22、Th17细胞、IL-17、IL-22和miR-200相对表达水平联合检测对系统性红斑狼疮合并感染患儿预后的AUC为0.897,敏感度为82.60%,特异度为81.70%,高于各指标单独检测(P<0.05)。结论Th22、Th17细胞及细胞因子在系统性红斑狼疮合并感染患儿中高表达,miR-200则低表达,上述指标检测能有效预测患儿预后,且联合预测价值最高。OBJECTIVE To investigate the levels of T helper cells(Th)22,Th17,cytokines and micrornas(miR)-200 in children with systemic lupus erythematosus co-infection,and to analyze the predictive value of each index.METHODS One hundred and fifty children with systemic lupus erythematosus(SLE)co-infection admitted to Shengjing Hospital Affiliated to China Medical University from Mar.2020 to Apr.2023 were selected as the infection group,150 children with SLE without infection admitted to the hospital during the same period were selected as the non-infection group,and 15o healthy children underwent health examination were selected as the control group.Children in the infection group were followed up for one year and divided into the good prognosis group and poor prognosis group according to whether there was organ damage.The relative expression levels of Th22,Th17 cells,interleukin-17,IL-22 and miR-20o in the three groups,the good and the poor prognosis group were compared.The predictive value of Th22,Th17 cells,IL-17,IL-22 and miR-200 levels in children with systemic lupus erythematosus co-infection was analyzed by receiver operating characteristic(ROC)curve.RESULTS The level of miR-200 in the infection group was(1.14±0.27),which was lower than in the non-infection group(1.36±0.33)and the control group(P<0.05).The levels of Th22,Th17 cells,IL-17 and IL-22 in the infection group were(6.19±1.24),(1.76±0.38),(21.16±4.48)and(52.79±7.94),respectively,which were higher than those in the non-infection group and the control group(P<0.05).The miR-200 in the poor prognosis group was(0.91±0.22),lower than that in the good prognosis group(P<0.05),and the Th22,Th17 cells and IL-17,IL-22 in the poor prognosis group were(6.86±1.35),(1.97±0.36),(24.36±5.03)and(58.51±7.45),respectively,higher than those in the good prognosis group(P<0.05).The AUC of the combined detection of relative expression levels of Th22,Th17 cells,IL-17,IL-22 and miR-200 for the prognosis of children with systemic lupus erythematosus co-infected was 0.897,w
