佳木斯市医疗卫生机构医生对分级诊疗制度的认知情况调查研究

摘　　要：目的了解佳木斯市各级医院医生对分级诊疗制度的认知情况,为分级诊疗制度的优化和长远发展提供借鉴。方法采用分层随机抽样的方法于2024年6月选取佳木斯市14家医疗卫生机构的420名医生作为调查对象。采用χ^(2)检验分析医生对基层首诊、双向转诊内涵的认知情况。结果在对分级诊疗工作内容的认知上,三级医院医生的总体认知较强,占84.95%;二级医院次之,占80.52%;社区卫生服务中心医生的总体认知最低,占80.02%。社区卫生服务中心医生正确把握基层首诊内涵的比例为78.24%,明显高于二级医院的61.32%及三级医院的73.23%,差异有统计学意义(χ^(2)=16.324,P<0.001)。结论在佳木斯市的医疗卫生体系中,医疗卫生机构医生对分级诊疗制度的核心概念、主要内容及内涵的认知程度相对较低,需进行强化提升。Objective To understand the cognition of graded diagnosis and treatment system among doctors at all levels in Jiamusi City,and provide a reference for the optimization and long-term development of the graded diagnosis and treatment system.Methods A total of 420 doctors from 14 medical and health institutions in Jiamusi City were selected by stratified random sampling in June 2024.Chi-square test was used to analyze the cognition of doctors on the connotation of first diagnosis at primary level and two-way referral.Results In terms of the cognition of hierarchical diagnosis and treatment,the overall cognition of doctors in tertiary hospitals was stronger,accounting for 84.95%;secondary hospitals were the second,accounting for 80.52%;the doctors of community health service center had the lowest overall cognition,accounting for 80.02%.The proportion of doctors in community health service center who correctly understood the connotation of first diagnosis at primary level was 78.24%,which was significantly higher than that in secondary hospitals(61.32%)and tertiary hospitals(73.23%),the difference was statistically significant(χ^(2)=16.324,P<0.001).Conclusion In the medical and health system of Jiamusi City,doctors in medical and health institutions have relatively low cognition of the core concept,main content and connotation of the hierarchical diagnosis and treatment system,which needs to be strengthened and improved.
