摘 要:目的 探讨多剂量甲硝唑联合保妇康栓治疗复发性滴虫性阴道炎感染的疗效,及治疗对辅助性T细胞1(Th1)/Th2细胞因子、阴道微生态的影响。方法 选取2022年1月-2024年3月赣州市人民医院收治的118例复发性滴虫性阴道炎感染患者为研究对象,根据随机数表法分为甲硝唑组和联合组,各59例。比较两组治疗效果、症状消退时间、Th1/Th2细胞因子水平及阴道微生态。结果 联合组治疗疗效优于甲硝唑组(P=0.005);联合组阴道充血、外阴瘙痒、外阴灼痛、白带异常、尿频尿痛症状消退时间分别为(8.43±1.19)、(8.15±1.21)、(5.69±0.94)、(10.34±1.37)、(6.23±1.22)d,短于甲硝唑组(P<0.05);治疗后联合组白细胞介素(IL)-2/IL10、IL-2/IL-4、肿瘤坏死因子-α(TNF-α)/IL-8、干扰素-γ(IFN-γ)/IL-4、IFN-γ/IL-10分别为(2.53±0.35)、(0.87±0.23)、(2.87±0.53)、(1.35±0.27)、(2.71±0.39),高于甲硝唑组(P<0.05);治疗后联合组阴道清洁度Ⅰ~Ⅱ级、阴道菌群密度Ⅱ~Ⅲ级、阴道菌群多样性Ⅱ~Ⅲ级、阴道pH值3.8~4.5、乳杆菌分级Ⅱa占比分别为81.36%、86.44%、86.44%、86.44%,62.71%高于甲硝唑组(P<0.05),白细胞酯酶阳性、过氧化氢酶阳性、阴道微生态异常分别为18.64%、15.25%、13.56%低于甲硝唑组(P <0.05);联合组药物不良反应与甲硝唑组比较,差异无统计学意义(P=0.542)。结论 多剂量甲硝唑联合保妇康栓治疗复发性滴虫性阴道炎感染可提高患者临床治疗效果,提高Th1/Th2细胞因子水平,调节阴道微生态平衡。OBJECTIVE To investigate the efficacy of multi-dose metronidazole combined with Baofukang Supposal in the treatment of recurrent trichomoniasis vaginitis and the impacts on T helper 1(Th1)/Th2 cytokines and va-ginal microecology.METHODS A total of 118 patients with recurrent trichomoniasis vaginitis treated in Ganzhou People's Hospital from Jan.2022 to Mar.2024 were selected as the study objects and divided into metronidazole group and combination group according to random number table method,with 59 cases in each group.The thera-peutic effect,symptom resolution time,Thl/Th2 cytokines and vaginal microecology were compared between these two groups.RESULTS The therapeutic effect of combination group was better than that in metronidazole group(P=0.005).The time for regression of symptoms including vaginal congestion,vulva pruritus,vulva burning pain,abnormal leukorrhea and frequent urination in combination group were(8.43±1.19),(8.15±1.21),(5.69±0.94),(10.34±1.37)and(6.23±1.22)days,respectively,whichwereshorter than thosein metronidazole group(P<0.05).After treatment,interleukin-2/IL-10,IL-2/IL-4,tumor necrosis factor-α(TNF-α)/IL-8,interferon-(IFN-γ)/IL-γ4and IFN-γ/IL-10in thecombination group were(2.53±0.35),(0.87±0.23),(2.87±0.5),(1.35±0.27)and(2.71±0.39),higher than thosein metronidazolegroup(P<O.05).Moreover,the percentages of vaginal cleanliness I to II,vaginal flora density II to II,Vaginal flora di-versity II to II,vaginal pH 3.8-4.5,and Lactobacillus grade II A of the combnation group were respectively 81.36%,86.44%,86.44%,86.44%and 62.71%,higher than those of the metronidazole group(P<0.05),leucocyte esterase the positive,positive catalase and abnormal vaginal microecology of the combination group were respectively 18.64%,15.25%and 13.56%,lower than those of the metronidazole group(P<0.05).There was no significant difference in adverse drug reactions between the combined group and the metronidazole group(P=0.542).CONCLUSION Multi-dose metronidazole combined with Baofukang Supposal in the tre
