下生殖道感染相关妊娠早期胚胎停育的性激素及自噬相关蛋白水平

摘　　要：目的探讨下生殖道感染致妊娠早期胚胎停育患者炎症指标及自噬相关蛋白等指标表达情况。方法选取2022年10月-2023年10月于内蒙古医科大学附属医院就诊的感染导致胚胎停育患者33例作为感染组,另选择同期正常早孕自愿要求终止妊娠者30例作为对照组,检测两组血清实验室生化指标、性激素、甲状腺激素、炎性指标,以及蜕膜组织的氧化应激指标、自噬相关蛋白表达水平。结果感染组白细胞(WBC)计数、C-反应蛋白(CRP)、降钙素原(PCT)分别为(9.66±1.61)×10^(9)/L、(10.5±2.01)mg/L、(3.38±0.81)ng/ml高于对照组(P<0.05);感染组人绒毛膜促性腺激素(HCG)与性激素六项卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)、泌乳素(PRL)、睾酮(T),以及甲状腺激素三项游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)表达水平低于对照组(P<0.05);感染组白细胞介素(IL)-2、IL-4、IL5、IL-13分别为(16.69±1.42)、(50.67±4.04)、(15.53±1.88)、(15.34±1.60)ng/L高于对照组(P<0.05);感染组蜕膜组织丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肤过氧化物酶(GSH-Px)分别为(10.42±0.95)nmol/L、(70.61±3.32)U/L、(428.55±14.63)U/L高于对照组(P<0.05);感染组蜕膜组织自噬相关蛋白Atg7、Beclin1、LAMP1、LC3Ⅰ/Ⅱ的相对表达水平分别为(1.26±0.15)、(1.77±0.13)、(1.54±0.14)、(1.07±0.09)高于对照组(P<0.05)。结论下生殖道感染会干扰母体的内分泌系统、免疫系统,导致激素与炎性因子水平异常变化,引发胚胎过度氧化应激与细胞自噬,或导致胚胎停育。OBJECTIVE To investigate the expression of inflammatory indexes and autophagy-related proteins in pa-tients with early embryonic diapause due to lower genital tract infections.METHODS Totally 33 patients with early embryonic diapause caused by lower genital tract infections who attended the Affiliated Hospital of Inner Mongolia Medical University from Oct.2022 to Oct.2023 were selected as the infection group,and 30 patients who volun-tarily requested for termination of early pregnancy during the same period were selected as the control group.ser-um biochemical indicators,sex and thyroid hormones,inflammatory indicators as well as oxidative stress indica-tors and autophagy-related protein levels of molting membrane tissue were detected in both groups.RESULTS The white blood cell(WBC)count,C-reactive protein(CRP),and procalcitonin(PCT)in the infection group were(9.66±1.61)×10^(9)/L,(10.5±2.01)mg/L,and(3.38±0.81)ng/ml,respectively,higher than those of the control group(P<0.05).While the expression levels of human chorionic gonadotropin(HCG),six sex hormones follicle stimulating hormone(FSH),luteinizinghormone(LH),estradiol(E2),progesterone(P),prolactin(PRL),testosterone(T),and three thyroid hormones free triodothyronine(FT3),free thyroxine(FT4),and thy-roid stimulating hormone(TSH)in the infection group were lower than those in the control group(P<0.05).In-terleukin(IL)-2,IL-4,IL5,and IL-13 in the infection group were(16.69±1.42),(50.67±4.04),(15.53±1.88),and(15.34±1.60)ng/L,respectively,higher than those in the control group(P<0.05).The malondial-dehyde(MDA),superoxide dismutase(SOD),and glutathione peroxidase(GSH-Px)of the molting membrane tis-sues in the infection group were(10.42±0.95)nmol/L,(70.61±3.32)U/L,and(428.55±14.63)U/L,respec-tively,higher than those in the control group(P<0.05);the relative expression levels of autophagy-related pro-teins Atg7,Beclinl,LAMP1,and LC3I/in the infection group were(1.26±0.15),(1.77±0.13),(1.540.14)and(1.07±0.09),respectively,higher than those in the control gr
