从卫气理论探讨时间节律与免疫系统对溃疡性结肠炎发病的影响

摘　　要：卫气的运行遵循“昼日行于阳,夜行于阴”的昼夜规律且具有“护卫周身”的防御功能。以“卫气理论”为指导,认为时间节律和免疫系统在溃疡性结肠炎(UC)的发病中具有重要作用。卫气运行失常,“阳不入阴”可导致UC发病;卫气巡行使疾病呈现出“旦慧、昼安、夕加、夜甚”的变化规律,这与免疫反应的节律性特征具有相似性。卫气的防御功能在UC肠道免疫中至关重要,卫气来源之脾胃是维持UC肠道黏膜免疫稳态的关键;同时卫气升降出入受阻,卫气内伐,邪伏于肠,发为UC。提出在“卫气理论”指导下治疗UC,包括择时饮食与择时治疗,以达到人与自然相统一;顾护脾胃的同时兼顾肺肾,平衡三焦营卫,以期提高UC的临床疗效。The movement of wei qi(defensive qi)follows the circadian rhythm of"circulating on the yang during the day,and on the yin at night"and serves a defensive function to"protect the body".Guided by the theory of wei qi,it is believed that time rhythms and the immune system play significant roles in the pathogenesis of ulcerative colitis(UC).Dysfunction in wei qi circulation,particularly when"yang fails to enter yin,"can lead to the onset of UC;the cyclical nature of wei qi's movement results in disease patterns characterized by"morning relief,daytime stability,evening aggravation,and nighttime worsening",which align with the rhythmic characteristics of immune responses.The defensive function of wei qi is crucial in maintaining intestinal immunity of patients with UC,and the spleen and stomach,which are the sources of wei qi,are key to sustaining intestinal mucosal immune homeostasis;additionally,obstruction in the ascending and descending movements of wei qi,internal disruption,and latent pathogen in the intestines lead to the development of UC.Based on the theory of wei qi,treatment approaches for UC are proposed,including time-based dietary adjustments and chronotherapy to harmonize human activities with natural rhythms;these approaches emphasize protecting the spleen and stomach while also considering the lungs and kidneys,balancing sanjiao,and harmonizing ying qi and wei qi,so as to improve the clinical effectiveness of UC treatment.
