2例抗NMDAR脑炎合并卵巢畸胎瘤患者的综合护理策略探究

摘　　要：目的探讨抗N-甲基-D-天冬氨酸受体(NMDAR)脑炎合并卵巢畸胎瘤患者的护理方法,提高护理效果。方法对2例抗NMDAR脑炎合并卵巢畸胎瘤患者进行针对性护理,包括中枢性低通气护理、精神异常护理、不自主运动伴有癫痫护理、自主神经功能障碍护理、畸胎瘤术后护理、药物不良反应监测与护理、营养支持并发胃肠道反应护理、健康教育及延续性护理等。结果经综合护理,患者1病情逐渐稳定,癫痫发作得到控制,意识状态有所恢复;患者2神志较前改善,不自主运动减少,近2月无癫痫发作,脑电图未见痫样放电,病情趋于平稳。结论针对抗NMDAR脑炎合并卵巢畸胎瘤患者的综合护理措施,能有效促进患者康复,减少并发症,提高生活质量。Objective To explore the nursing methods for patients with anti-N-methyl-D-aspartate receptor(NMDAR)encephalitis complicated with ovarian teratoma and improve nursing outcomes.Method Targeted nursing was provided to 2 patients with anti-NMDAR encephalitis complicated with ovarian teratoma,encompassing central hypoventilation nursing,nursing for psychiatric abnormalities,nursing for involuntary movements accompanied by seizures,nursing for autonomic nervous dysfunction,postoperative nursing for teratoma,monitoring and nursing for adverse drug reactions,nursing for nutrition support complicated by gastrointestinal reactions,health education,and continuous nursing.Result After comprehensive nursing,patient 1 experienced gradual stabilization of her condition,controlled seizures,and improved consciousness;patient 2 demonstrated improved mental status,reduced involuntary movements,no seizures in the past two months,no epileptiform discharges on electroencephalogram,and her condition tended to stabilize.Conclusion Comprehensive nursing measures for patients with anti-NMDAR encephalitis complicated with ovarian teratoma can effectively promote patient recovery,reduce complications,and improve quality of life.
