张培彤应用“培土生金法”治疗肺结节术后经验撷英

摘　　要：肺结节是近年来的常见疾病,发病率和检出率逐年增高。目前西医对于肺结节多采用定期复查随访和手术治疗为主,治疗手段存在较大的局限性,对于尚未达到手术标准的患者没有有效的治疗策略,对于手术后的患者也没有进一步的诊疗方案。张培彤教授治疗肺结节临证30余年,经验丰富。张培彤教授认为肺结节为本虚标实证,标在邪聚,本于正伤,而手术则可以切除局部凝聚的实邪,但也会产生手术相关病理因素,并进一步损伤人体正气,因而在治疗肺结节术后时以“扶正”为主。培土生金法,又称补脾益肺法,即通过健脾气以补肺气的治疗方法,遵循“虚则补其母”的理论思想,是五行学说体现在临床中的一种诊疗方法。张培彤教授临证治疗肺结节术后患者时,多以培土生金法为核心治则,实行辨病和辨证相结合的个体化治疗,临床疗效确切,现总结经验如下。Pulmonary nodule is a common disease in recent years,and its incidence rate and detection rate are increasing year by year.At present,Western medicine mainly adopts regular follow-up and surgical treatment for pulmonary nodules,with significant limitations in treatment methods.At present,Western medicine mainly adopts regular follow-up and surgical treatment for pulmonary nodules,with significant limitations in treatment methods.There is no effective treatment strategy for patients who have not yet met surgical standards,and there is no further diagnosis and treatment plan for postoperative patients.Professor ZHANG Peitong has over 30 years of clinical experience in treating pulmonary nodules.Professor ZHANG Peitong believes that pulmonary nodules are characterized by deficiency in origin and excess in superficiality,with the evil gathering in superficiality and body injury in origin.Surgery can remove locally condensed solid pathogens,but it also can produce surgical related pathological factors and further damage the body′s vital energy.Therefore,in the treatment of pulmonary nodules after surgery,the main approach is to“support the vital energy”.The method of cultivating soil to generate gold,also known as the method of nourishing spleen and benefiting lung,is a treatment method that strengthens spleen and Qi to replenish lung.It follows the theoretical idea of“reinforcing the mother-organ to treat deficiency”and is a diagnostic and therapeutic method of the Five Elements Theory in clinical practice.Professor ZHANG Peitong′s clinical treatment of postoperative patients with pulmonary nodules often focuses on the principle of cultivating soil to generate gold,and implements personalized treatment that combines disease differentiation and syndrome differentiation.The clinical efficacy is accurate,and the experience summarized was as follows.
