基于大气下陷理论论治慢性阻塞性肺疾病

摘　　要：慢性阻塞性肺疾病是一种以慢性咳嗽、咯痰、胸闷气短、喘息和(或)呼吸困难为症状的疾病。“大气”首见于《黄帝内经》,张锡纯探究其原意,首提“大气下陷”理论,以益气升提之法治疗下陷之证。基于“大气下陷”理论角度,发现“大气下陷”与慢性阻塞性肺病的发生、发展密切相关,是因其肺、脾、肾三脏虚损,进而产生痰、瘀,甚则出现虚极耗散元气的危急情况。治疗上应重视大气虚损下陷之本,兼顾痰瘀之标,治以益气升提,化痰祛瘀,在病情危重之时,予以补益收敛之法,以此对慢性阻塞性肺疾病患者积极施治。Chronic obstructive pulmonary disease is a condition consisting of chronic cough,expectoration,shortness of breath,wheezing,and(or)dyspnea.“Qi”was first seen in Huangdi Neijing.ZHANG Xichun explored the original meaning of the book,first proposed the theory of“pectoral Qi sinking”,to treat Qi sinking by improving Qi and lifting.Based on the theory of“pectoral Qi sinking”,it is found that“pectoral Qi sinking”is closely related to the occurrence and development of chronic obstructive pulmonary.It is because the deficiency of lung,spleen and kidney,and then produces phlegm and blood stasis and even the critical situation.In terms of treatment,the attention should be paid to the root of pectoral Qi sinking,giving consideration to phlegm and blood stasis.The treatment is to improve Qi and lift,to remove phlegm and blood stasis.When the condition is critical,the method of tonifying and converging should be given,so as to actively treat patients with chronic obstructive pulmonary disease.
