桑白皮汤加减治疗慢性阻塞性肺疾病急性加重期患者的临床研究

摘　　要：目的探究桑白皮汤加减对慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者疗效、血清白细胞介素-37(interleukin-37,IL-37)、白细胞介素-8(interleukin-8,IL-8)水平及血氧参数的影响。方法选取2020年2月—2023年1月医院收治的100例AECOPD患者为研究对象,按照随机数字法分为对照组和试验组,各50例。对照组采用西药治疗,试验组在对照组基础上联合桑白皮汤加减治疗,两组均治疗2周。对比治疗后肺功能,治疗前后中医证候积分、血氧参数及血清炎性指标(IL-37、IL-8)水平,分析治疗期间不良反应发生情况。结果治疗后,试验组各肺功能指标高于对照组(P<0.05);治疗后,两组各中医证候积分、二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))、血清IL-37与IL-8水平较治疗前降低,且试验组低于对照组(P<0.05);两组氧分压(partial pressure of oxygen,PaO_(2))、氧合指数(oxygenation index,SpO_(2))较治疗前升高,且试验组高于对照组(P<0.05),试验组不良反应发生率为8.00%(4/50),与对照组10.00%(5/50)比较差异无统计学意义(P>0.05)。结论桑白皮汤加减能有效改善AECOPD患者临床症状,下调血清IL-37与IL-8水平,改善肺功能与血氧参数。Objective To explore the effect of Modified Sangbaipi Decoction(桑白皮汤加减)on the efficacy,serum levels of interleukin-37(IL-37),interleukin-8(IL-8)and blood oxygen parameter in patients with AECOPD.Methods 100 patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)admitted to the hospital were selected from February 2020 to January 2023,and were divided into control group(50 cases)and experimental group(50 cases)according to the random number method.The control group received western medicine treatment,and the experimental group was given Modified Sangbaipi Decoction on the basis of the control group,and both groups were treated for 2 weeks.The pulmonary function after treatment and TCM syndromes scores,blood oxygen parameters and serum inflammatory indicators(IL-37,IL-8)before and after treatment were compared,and the adverse reactions were analyzed during treatment.Results After treatment,the pulmonary function indicators were higher in the experimental group than those in control group(P<0.05).The TCM syndromes scores,PaCO_(2),serum levels of IL-37 and IL-8 in the two groups after treatment were declined compared with those before treatment,and the above indicators in the experimental group were lower than those in the control group(P<0.05).The values of PaO_(2) and SpO_(2) in both groups were risen compared to those before treatment,and the two indicators in the experimental group were higher(P<0.05).The incidence rate of adverse reactions in the experimental group was 8.00%(4/50),which was not statistically different compared with 10.00%(5/50)in the control group(P>0.05).Conclusion Modified Sangbaipi Decoction can effectively improve the clinical symptoms,down-regulate the levels of serum IL-37 and IL-8,and improve the pulmonary function and blood oxygen parameters in patients with AECOPD.
