补肾活血汤辅助治疗多囊卵巢综合征致不孕症的效果分析

摘　　要：目的分析补肾活血汤辅助治疗多囊卵巢综合征(polycystic ovary syndrome,PCOS)致不孕症的效果。方法选取199例2020年3月—2022年8月防城港市妇幼保健院收治的PCOS致不孕症患者,应用随机数字表法分为对照组(99例)和研究组(100例)。对照组采用克罗米芬治疗,研究组在对照组的基础上采用补肾活血汤治疗,两组连续治疗3个月经周期,均于治疗后随访6个月。比较两组治疗后临床疗效及随访期间妊娠情况,治疗前后中医证候积分、子宫内膜厚度及卵巢功能、子宫及卵巢血流情况。结果治疗后,研究组总有效率高于对照组;治疗后两组中医证候各项积分、卵泡数、卵巢体积、子宫搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)及卵巢RI降低,研究组更低;两组子宫内膜厚度、卵泡直径升高,研究组更高;两组卵巢PI降低(P<0.05),但两组间治疗后卵巢PI比较,差异无统计学意义(P>0.05);随访期间研究组妊娠率、排卵率更高,流产率更低(P<0.05)。结论补肾活血汤辅助治疗PCOS致不孕症,有助于缓解患者的临床症状及体征,改善子宫内膜厚度及卵巢功能,优化子宫及卵巢血流情况,提高临床疗效及妊娠率、排卵率。Objective To analyze the effect of Bushen Huoxue Decoction(补肾活血汤)on infertility caused by polycystic ovary syndrome(PCOS).Methods A total of 199 patients with PCOS infertility admitted to Fangchenggang Maternal and Child Health Hospital were selected from March 2020 to August 2022 and divided into the control group(99 cases)and the research group(100 cases).The grouping method was the random number table method.The control group was treated with clomiphene citrate,and the research group was treated with Bushen Huoxue Decoction based on the control group.The two groups were treated for three menstrual cycles continuously and were followed up for six months after treatment.The clinical efficacy after treatment,pregnancy during follow-up,TCM syndrome scores,endometrial thickness and ovarian function,uterine and ovarian blood flow before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the research group was higher than that of the control group.After treatment,compared with those before treatment,TCM syndrome scores,the number of follicles,ovarian volume,pulsatility index(PI),resistance index(RI)of uterus and RI of ovary of the two groups decreased,and those of the research group were lower than those of the control group.After treatment,compared with those before treatment,the endometrial thickness and follicular diameter of the two groups increased,and those of the research group were higher than those of the control group.After treatment,compared with those before treatment,the ovarian PI of the two groups decreased(P<0.05),but there was no difference between the two groups after treatment(P>0.05).The pregnancy rate and ovulation rate of the research group were higher,and the abortion rate was lower during the follow-up period(P<0.05).Conclusion Bushen Huoxue Decoction can help to relieve the clinical symptoms and signs,improve endometrial thickness and ovarian function,optimize uterine and ovarian blood flow,and improve clinical efficacy,pregn
