化浊疏肝祛瘀法联合穴位贴敷治疗早期糖尿病患者疗效观察

摘　　要：目的探讨化浊疏肝祛瘀法联合穴位贴敷治疗早期糖尿病(prediabetes for dummies,PDM)患者的疗效。方法选取2022年6月—2023年6月张家口市中医院内分泌科收治的120例PDM患者,采用随机数表法分为对照组(生活方式干预)和研究组(生活方式干预+化浊疏肝祛瘀汤+穴位贴敷)各60例,均持续干预4周。比较两组临床疗效和干预前后的中医证候积分、空腹血糖(fasting plasma glucose,FPG)、餐后2 h血糖(2 h postprandial blood glucose,2 h PG)、血红蛋白(glycosylated hemoglobin,HbA1c)、体质量指数(body mass index,BMI)、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、谷氨酸氨基转移酶(alanine aminotransferase,ALT)、天冬氨酸转氨酶(aspartate transaminase,AST)、血肌酐(serum creatinine,SCr)水平及治疗期间不良事件发生率。结果研究组总有效率为86.7%(52/60),高于对照组的60.0%(36/60)(P<0.05);干预后,研究组眩晕头痛、胸闷呕吐和心悸失眠积分低于对照组(P<0.05);研究组FPG、2 h PG、HbA1c和BMI均低于对照组(P<0.05);研究组TC、TG、LDL-C和ALT、AST、SCr水平明显低于对照组,HDL-C水平高于对照组(P<0.05)。结论化浊疏肝祛瘀法联合穴位贴敷治疗PDM效果理想,可有效改善其糖脂代谢和肝肾功能。Objective To explore the curative effect of dispelling turbidity-soothing liver-removing blood stasis method combined with acupoint application in patients with prediabetes for dummies(PDM).Methods A total of 120 patients with PDM admitted to endocrinology department of Zhangjiakou Hospital of Traditional Chinese Medicine were enrolled between June 2022 and June 2023.They were divided into study group(60 cases)and control group(60 cases).The control group was given lifestyle intervention,whilethe study group was additionally given dispelling turbidity-soothing liver-removing blood stasis method combined with acupoint application for 4 weeks.The clinical curative effect,scores of TCM syndromes andthe levels of fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1c),body mass index(BMI),total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),aspartate transaminase(AST),alanine aminotransferase(ALT)and serum creatinine(SCr)before and after intervention,and the incidence of adverse events during treatment were compared between the two groups.Results The total response rate of the study groupwas higher thanthose of the control group[86.7%(52/60)vs 60.0%(36/60),P<0.05].Compared to those of the control group after intervention,the scores of TCM syndromes(dizziness and headache,chest distress and vomiting,palpitation and insomnia),the levels of FPG,2 h PG,HbA1,BMI and the levels of TC,TG,LDL-C,ALT,AST and SCr in the study group were lower(P<0.05),while HDL-C level was higher(P<0.05).Conclusion Dispelling turbidity-soothing liver-removing blood stasis method combined with acupoint application has significant effect on PDM,and can effectively improve glucose-lipid metabolism and liver-kidney function.
