炙甘黄白汤联合西替利嗪在脾肺气虚型过敏性鼻炎中的应用分析

摘　　要：目的探究自拟方剂炙甘黄白汤联合西替利嗪对脾肺气虚型过敏性鼻炎患者血清炎症因子及微量元素水平的影响。方法收集2022年3月—2023年2月唐山市中医医院耳鼻喉科收治的124例脾肺气虚型过敏性鼻炎患者,随机分为试验组和对照组,各62例。对照组予以盐酸左西替利嗪分散片,试验组予以自拟方剂炙甘黄白汤联合西替利嗪。两组患者均以4周为1个疗程,共治疗1个疗程。对比两组临床疗效、症状积分、血清炎症因子、微量元素及不良反应发生情况。结果与对照组比较,试验组临床总有效率较高(P<0.05)。与治疗前比较,两组治疗后鼻痒、鼻塞、喷嚏、擤鼻及症状总积分显著降低(P<0.05),且与对照组比较,试验组积分更低(P<0.05)。治疗后,两组血清白细胞介素-4(interleukin-4,IL-4)、白细胞介素-5(interleukin-5,IL-5)、白细胞介素-10(interleukin-10,IL-10)、白细胞介素-17(interleukin-17,IL-17)水平降低(P<0.05);且与对照组比较,试验组水平更低(P<0.05)。治疗后,两组锌(Zn)水平升高,铜(Cu)、锰(Mn)水平降低(P<0.05);且与对照组比较,试验组Zn水平更高,Cu、Mn水平更低(P<0.05)。与对照组比较,试验组不良反应发生率较低,差异具有统计学意义(P<0.05)。结论自拟方剂炙甘黄白汤联合西替利嗪对脾肺气虚型过敏性鼻炎患者疗效良好,能够有效缓解患者临床症状,其作用机制可能与调节机体内微量元素水平、抑制炎症反应具有密切相关性。Objective To explore the effect of self-made Zhigan Huangbai Decoction(炙甘黄白汤)combined with cetirizine on the levels of serum inflammatory factors and trace elements in patients with allergic rhinitis of spleen-lung Qi deficiency type.Methods A total of 124 patients with allergic rhinitis of spleen-lung Qi deficiency type admitted to the otorhinolaryngology department of the hospital from March 2022 to February 2023 were collected and randomly divided into the test group and the control group,with 62 cases in each group.The control group was given levocetirizine hydrochloride dispersible tablets,while the experimental group was given self-made Zhigan Huangbai Decoction combined with cetirizine.Both groups were treated for a total of 1 course,with a duration of 4 weeks.The clinical efficacy,symptom scores,serum inflammatory factors,trace elements,and incidence of adverse reactions were compared between the two groups.Results After treatment,compared with the control group,the experimental group had a higher clinical total effective rate(P<0.05).After treatment,compared with those before treatment,the total scores of nasal itching,nasal congestion,sneezing and blowing nosein the two groups were significantly reduced(P<0.05)and the experimental group had significantly lower total scores compared with those of the control group(P<0.05).After treatment,the serum levels of interleukin-4(IL-4),interleukin-5(IL-5),interleukin-10(IL-10)and interleukin-17(IL-17)in two groups were decreased compared with those before treatment(P<0.05)and the levels in the experimental group were significantly lower than those of the control group(P<0.05).After treatment,compared with those before treatment,the Zn levels in both groups increased,while the levels of Cu and Mn decreased(P<0.05).Compared with the control group,the experimental group had higher levels of Zn and lower levels of Cu and Mn(P<0.05).Compared with that of the control group,the incidence of adverse reactions in the experimental group was lower,and the differen
