合营止痛汤加减治疗胸腰椎骨折患者PVP术后疼痛疗效观察

摘　　要：目的探讨合营止痛汤加减在胸腰椎骨折患者经皮椎体成形(percutaneous vertebroplasty,PVP)术后疼痛治疗中的疗效。方法选取医院2021年6月—2022年12月收治的104例PVP术后患者作为研究对象,按照随机数字表法分为对照组(52例,采用艾瑞昔布治疗)和观察组(52例,采用合营止痛汤加减治疗),治疗时间为2周。观察两组患者治疗后镇痛效果、疼痛情况、中医证候积分和不良反应。结果治疗后2周,观察组临床镇痛总有效率为98.08%(51/52),高于对照组84.62%(44/52)(P<0.05)。两组疼痛评分组间、时间点及交互差异均有统计学意义(P<0.001)。观察组术后1、2周疼痛评分均显著低于对照组(P<0.05)。治疗2周后,两组腰背部刺痛、痛点固定、皮肤肿胀,大便干结等症状积分均低于治疗前(P<0.05);且观察组腰背部刺痛、痛点固定、皮肤肿胀、大便干结等症状积分均低于对照组(P<0.05)。治疗2周后,观察组患者胃肠道不适、乏力、皮疹、肝肾功能损伤等不良反应发生率低于对照组(P<0.05)。结论合营止痛汤加减治疗PVP术后疼痛患者能够降低疼痛感,改善中医疼痛症状,安全性较高。Objective To explore the therapeutic effect of Modified Heying Zhitong Decoction(合营止痛汤加减)in the treatment of pain after percutaneous vertebroplasty(PVP)for patients with thoracolumbar fractures.Methods A total of 104 patients with postoperative pain after PVP admitted to the hospital from June 2021 to December 2022 were selected as research subjects.They were randomly divided into a control group(52 cases,treated with lornoxicam)and an observation group(52 cases,treated with modified joint venture analgesic decoction)according to a random number table method.The treatment period was 2 weeks.The analgesic effect,pain status,TCM symptom scoresand adverse reactions of the two groups were observed after treatment.Results Two weeks after treatment,the total effective rate of clinical analgesia in the observation group was 98.08%(51/52),higher than 84.62%(44/52)in the control group(P<0.05).There were statistically significant differences in pain scores between the two groups in terms of time point and interaction(P<0.001).The pain scores in the observation group were significantly lower than those in the control group at 1 and 2 weeks after surgery(P<0.05).Two weeks after treatment,the scores of symptoms such as back pain,fixed pain points,skin swellingand dry stool in both groups were lower than those before treatment(P<0.05).In addition,the observation group had lower scores of symptoms such as sharp pain in the back,fixed pain points,skin swellingand dry stool than the control group(P<0.05).After 2 weeks of treatment,the incidence of adverse reactions such as gastrointestinal discomfort,fatigue,rash,liver and kidney function damage in the observation group was lower than that in the control group(P<0.05).Conclusion Modified Heying Zhitong Decoction can reduce pain sensation,improve pain symptoms in traditional Chinese medicine and has high safety in the treatment of postoperative pain patients with PVP.
