辛夷散联合氦氖激光治疗鼻-鼻窦炎疗效观察

摘　　要：目的探究辛夷散联合氦氖激光治疗对慢性鼻-鼻窦炎(chronic rhinosinusitis,CRS)患者的治疗疗效。方法选取2020年5月—2022年5月广州中医药大学东莞医院耳鼻咽喉科CRS患者114例,随机分为观察组(59例)和对照组(55例)。观察组接受辛夷散联合氦氖激光治疗,对照组接受氦氖激光治疗。通过比较两组患者的疗效、中医证候积分、鼻黏膜纤毛传输功能指标[鼻黏膜纤毛清除率(cilia clearance rate,MCC)和鼻黏膜纤毛输送率(mucociliary transport rate,MTR)]以及机体免疫功能指标[Toll样受体9(Toll-like receptor 9,TLR-9)]的变化。同时此用Lun-Kennedy、Lund-Mackay量表评估两组患者客观症状变化,并分析治疗效果和影响机制。结果治疗结束后,观察组、对照组的治疗总有效率分别为94.92%(56/59)、81.82%(45/55),观察组较高(P<0.05)。治疗后,观察组Lun-Kennedy、Lund-Mackay评分以及各项中医证候积分均低于对照组,观察组MTR、MCC高于对照组,差异具有统计学意义(P<0.05)。观察组TLR-9高于对照组,差异具有统计学意义(P<0.05)。观察组不良反应总发生率为8.47%(5/59),与对照组(3.64%,2/55)比较差异无统计学意义(P>0.05)。结论辛夷散联合氦氖激光治疗对CRS具有显著的治疗疗效,可以有效改善患者的症状。该治疗对患者的鼻黏膜纤毛传输功能和机体免疫功能有积极影响,通过调节TLR-9的水平,可能改善CRS预后。Objective To explore the therapeutic effect of Xinyi Powder(辛夷散)combined with He-Ne laser on patients with chronic rhinosinusitis(CRS).Methods Between May 2020 and May 2022,114 patients with CRS were randomly selected in the Department of Otolaryngology of Dongguan Hospital of Guangzhou University of Chinese Medicine,who were randomly divided into observation group(59 cases)and control group(55 cases).The observation group received Xinyi Powder combined with He-Ne laser treatment,and the control group received He-Ne laser treatment.The efficacy,TCM syndrome scores,nasal ciliary transport function index[cilia clearance rate(MCC)and mucociliary transport rate(MTR)]and immune function index[Toll-like receptor 9(TLR-9)]of the two groups were compared.At the same time,the objective symptom changes of the two groups were evaluated by Lun-Kennedy and Lund-Mackay scales,and the therapeutic effect and influencing mechanism were analyzed.Results After treatment,the total effective rate of the observation group and the control group was 94.92%(56/59)and 81.82%(45/55),respectively,and the total effective rate was higher in the observation group(P<0.05).After treatment,the scores of Lun-Kennedy,Lund-Mackay and various TCM syndrome scores in the observation group were lower than those in the control group,while the values of MTR and MCC in the observation group were higher than those in the control group,with statistical significance(P<0.05).The serum level of TLR-9 was higher than those in the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions was 8.47%(5/59)in the observation group and 3.64%(2/55)in the control group(P>0.05).Conclusion Xinyi Powder combined with He-Ne laser has a significant therapeutic effect on CRS,and can effectively improve the symptoms of patients.The treatment has positive effects on the nasal mucociliary transport function and immune function of patients,and may improve the prognosis of CRS by regulating the levels of TLR-9.
