自拟活血化瘀利水方联合体外冲击波对急性踝关节扭伤肿胀疼痛及炎症因子的影响

摘　　要：目的观察自拟活血化瘀利水方联合体外冲击波(extracorporeal shock wave,ESW)对急性踝关节扭伤肿胀疼痛及炎症因子的影响。方法选取108例自2020年2月—2023年2月医院收治的急性踝关节扭伤患者,随机平均分为两组。其中54例为观察组,采用自拟活血化瘀水方联合体外冲击波治疗;另54例为对照组,采用体外冲击波治疗。比较治疗前后踝关节中医证候积分变化、踝关节功能状况[Baird-Jackson踝关节评分、美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)踝-后足评分]、血清炎症因子[白细胞介素-1β(interleukin-1β,IL-1β)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子ɑ(tumor necrosis factor-ɑ,TNF-ɑ)]变化及治疗后恢复时间和治疗效果。结果治疗后,观察组中医证候积分较对照组明显下降(P<0.05);观察组踝关节Baird-Jackson评分和AOFAS评分较对照组明显下降(P<0.05);观察组IL-1β、IL-6和TNF-ɑ较对照组明显下降(P<0.05);观察组恢复时间如疼痛改善时间、肿胀缓解时间均较对照组明显下降(P<0.05);观察组治疗总有效率明显高于对照组(P<0.05)。结论活血化瘀利水方联合ESW治疗急性踝关节可提高治疗效果,缩短恢复时间,减轻炎症反应,改善踝关节功能。Objective To observe the effects of Self-Made Huoxue Huayu Lishui Decoction(自拟活血化瘀利水方)combined with extracorporeal shock wave(ESW)on swelling pain and inflammatory factors in acute ankle sprain.Methods A total of 108 patients with acute ankle sprain admitted to the hospital were enrolled and randomly divided into observation group(54 cases,Self-made Huoxue Huayu Lishui Decoction combined with ESW)and control group(54 cases,ESW)between February 2020 and February 2023.The changes in scores of TCM syndromes,ankle function status[Baird-Jackson ankle joint score,American Orthopedic Foot and Ankle Society(AOFAS)score]and serum inflammatory factors[interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-ɑ(TNF-ɑ)]before and after treatment,recovery time after treatment and curative effect were compared between the two groups.Results After treatment,the scores of TCM syndromes in the observation group were significantly lower than those inthe control group(P<0.05),the scores of ankle Baird-Jackson and AOFAS were significantly lower than those in the control group(P<0.05),the levels of IL-1β,IL-6 and TNF-ɑwere significantly lower than those in the control group(P<0.05),the recovery time(pain improvement time,swelling relief time)was significantly shorter than that in the control group(P<0.05),and the total response rate of treatment was significantly higher than that inthe control group(P<0.05).Conclusion Huoxue Huayu Lishui Decoction combined with ESW can improve the curative effect,shorten recovery time,relieve inflammatory response and improve ankle function in acute ankle sprain.
