中药温阳化气行水塌渍联合微波热疗治疗癌性腹水患者临床观察

摘　　要：目的观察中药温阳化气行水塌渍联合微波热疗治疗癌性腹水患者的临床效果。方法选取就诊于张家口市中医院的102例癌性腹水患者作为研究对象,随机分成对照组和治疗组,每组51例。对照组给予静脉化疗后实施局部微波热疗,治疗组在对照组的基础上同时予以中药塌渍法进行治疗。治疗2个周期后,比较两组患者在治疗后中医证候疗效、临床疗效、生活质量改善[Karnofsky功能学状态评分(KPS)]及治疗期间不良反应发生率,观察治疗前后肿瘤标志物水平变化。结果治疗2个周期后,治疗组患者在中医证候疗效有效率、临床客观缓解率和疾病控制率、生活质量总改善率均明显高于对照组患者,差异有统计学意义(P<0.05);治疗组患者肿瘤标志物糖类抗原199(carbohydrate antigen 199,CA199)、癌胚抗原(carcinoembryonic antigen,CEA)、糖类抗原125(carbohydrate antigen 125,CA125)水平均低于对照组患者,差异有统计学意义(P<0.05);两组患者不良反应发生情况比较,差异无统计学意义(P>0.05)。结论中药温阳化气行水塌渍联合微波热疗对癌性腹水临床疗效突出,能有效缓解临床症状并提高患者生活质量,抑制肿瘤标志物表达,且安全性良好,可在临床推广应用。Objective To observe the clinical effect of Chinese medicine collapse stains for warming Yang to transform Qi and activating water combined with microwave hyperthermia in the treatment of patients with malignant ascites.Methods A total of 102 patients with malignant ascites admitted to Zhangjiakou Hospital of Traditional Chinese Medicine were selected as the study subjects and were randomly divided into control group and treatment group with 51 cases in each group.The control group was treated with local microwave hyperthermia after intravenous chemotherapy,and the treatment group was given Chinese medicine collapse stains for warming Yang to transform Qi and activating water on the basis of the control group.After 2 cycles of treatment,the efficacy of TCM syndrome,clinical efficacy and improvement of quality of life[Karnofsky performance status score(KPS)]after treatment and incidence rates of adverse reactions during treatment were compared between the two groups,and the changes in tumor markers before and after treatment were observed.Results After 2 cycles of treatment,the effective rate of TCM syndrome efficacy,clinical objective remission rate,disease control rate and total improvement rate of quality of life in the treatment group were significantly higher than those in the control group(P<0.05).The levels of tumor markers such as carbohydrate antigen 199(CA199),carcinoembryonic antigen(CEA)and carbohydrate antigen 125(CA125)were significantly lower in the treatment group than those in the control group(P<0.05).Adverse events occurred in both groups during treatment and were relatively mild.After treatment of adverse reactions,the patients could continue to receive treatment without affecting the course of this observation,but there was no statistical difference between the two groups(P>0.05).Conclusion Chinese medicine collapse stains for warming Yang to transform Qi and activating water combined with microwave hyperthermia has outstanding clinical efficacy in the treatment of malignant ascites,and it can
