归芪白术方辅助化疗在中晚期胃癌中的应用效果

摘　　要：目的探讨归芪白术方辅助化疗在中晚期胃癌中的应用效果。方法以随机数字表法将2021年8月—2022年12月在该院接受治疗的127例中晚期胃癌分为Ⅰ组(63例)与Ⅱ组(64例)。Ⅰ组采取常规化疗治疗,Ⅱ组在Ⅰ组基础上使用归芪白术方辅助治疗,两组均治疗1个化疗周期并随访6个月。比较两组治疗后6个月临床疗效、治疗前及治疗后6个月生活质量评分、功能状态评分、中医证候积分、免疫功能、肿瘤标志物水平、治疗期间不良反应发生率。结果与Ⅰ组比较,Ⅱ组治疗后6个月总有效率更高;与治疗前比较,治疗后6个月两组欧洲癌症研究与治疗组织生命质量测定量表(European organization for research and treatment of cancer,EORTC QLQ-C30)、卡氏功能状态评分标准(Karnofsky functional status,KPS)评分、血清CD_(3)^(+)、CD_(4)^(+)、自然杀伤细胞(natural killer cells,NK)水平均上升,Ⅱ组高于Ⅰ组;与治疗前比较,治疗后6个月两组中医证候积分、血清CD_(8)^(+)、细胞角蛋白19片段21-1(cytokeratin 19 fragment 21-1,CYFRA21-1)、癌抗原19-9(cancer antigen 19-9,CA19-9)、癌抗原125(cancer antigen 125,CA125)、组织多肽特异抗原(tissue polypeptide specific antigen,TPS)水平均降低,Ⅱ组低于Ⅰ组;与Ⅰ组比较,治疗期间Ⅱ组各项不良反应发生率均更低,差异具备统计学意义(P<0.05)。结论归芪白术方辅助化疗在中晚期胃癌治疗中可有效提高临床疗效,增强免疫功能,降低肿瘤标志物表达水平,缓解临床症状,提升患者生活质量及功能状态,降低不良反应发生风险。Objective To explore the application effect of Guiqi Baizhu Recipe(归芪白术方)in adjuvant chemotherapy for advanced gastric cancer.Methods A total of 127 cases of advanced gastric cancer admitted to the hospital from August 2021 to December 2022 were divided into the groupⅠ(63 cases)and the groupⅡ(64 cases)by the random number table method.The groupⅠwas treated with routine chemotherapy,and the groupⅡwas treated with Guiqi Baizhu Recipe on the basis of the groupⅠ.Both groups were treated for 1 chemotherapy cycle and followed up for 6 months.6 months after treatment,the clinical efficacy,quality of life score,functional status score,TCM syndrome scores,immune function,tumor marker level before and 6 months after treatment,the incidence of adverse reactions during treatment were compared between the two groups.Results Compared with that of the groupⅠ,the total effective rate of the groupⅡwas higher.Compared with those before treatment,the quality of life measurement scale for European organization for research and treatment of cancer(EORTC QLQ-C30),Karnofsky functional status score(KPS),serum CD_(3)^(+),CD_(4)^(+) and natural killer cells(NK)levels of the two groups all increased 6 months after treatment,and those of the groupⅡwere higher than those of the groupⅠ.Compared with those before treatment,the TCM syndrome scores,serum CD_(8)^(+),cytokeratin 19 fragment 21-1(CYFRA21-1),cancer antigen 19-9(CA19-9),cancer antigen 125(CA125)and tissue polypeptide specific antigen(TPS)of the two groups decreased after treatment for 6 months,and the scores and levels of the groupⅡwere lower than those of the groupⅠ.Compared with that of the groupⅠ,the incidence of adverse reactions of the groupⅡwas lower during treatment.The differences were significant(P<0.05).Conclusion Guiqi Baizhu Recipe can effectively improve the clinical efficacy in the treatment of advanced gastric cancer,enhance the immune function,reduce the expression levels of tumor markers,relieve the clinical symptoms,improve the qu
