益肾化瘀降浊方对糖尿病肾病患者血管内皮功能和外周血自噬相关蛋白表达的影响

摘　　要：目的研究益肾化瘀降浊方对糖尿病肾病(diabetic nephropathy,DN)患者血管内皮功能和外周血自噬相关蛋白表达的影响。方法选取2018年6月—2021年6月在该院就诊的62例DN患者,按随机数字表法分为试验组和对照组各31例。对照组采用常规药物治疗,试验组在此基础上采用益肾化瘀降浊方治疗,比较两组患者糖代谢、肾功能、血管内皮功能、自噬相关蛋白等指标。结果治疗后,试验组总有效率高于对照组(P<0.05);治疗后,两组患者空腹血糖(fasting plasma glucose,FPG)、餐后2 h血糖(2 h postprandial plasma glucose,2 h PG)、糖化血红蛋白(glycated hemoglobin,HbA1c)水平均有不同程度下降,且试验组FPG、2 h PG、HbA1c水平低于对照组(P<0.05);治疗后,两组患者血肌酐(serum creatinine,SCr)、血尿素氮(blood urea nitrogen,BUN)、24 h尿总蛋白(24 h urine total protein,U-TP)水平均有不同程度下降,且试验组SCr、BUN、U-TP水平低于对照组(P<0.05);治疗后,试验组一氧化氮(nitric oxide,NO)水平高于对照组,内皮素-1(endothelin-1,ET-1)、血管性血友病因子(von Willebrand factor,vWF)水平低于对照组(P<0.05);治疗后,两组患者Beclin1、自噬相关蛋白7(autophagy related protein 7,ATG7)水平均有不同程度升高,且试验组Beclin1、ATG7水平高于对照组(P<0.05)。结论益肾化瘀降浊方联合常规西医治疗DN患者,明显提高治疗有效率,降低血糖水平,提高肾功能自噬相关蛋白表达,改善血管内皮功能。Objective To study the effect of Yishen Huayu Jiangzhuo Recipe(益肾化瘀降浊方)on vascular endothelial function and expression of autophagy related proteins in peripheral blood in patients with diabetes nephropathy(DN).Methods A total of 62 patients with DN who were treated in the hospital from June 2018 to June 2021 were selectedand divided into the experimental group and the control group by random number table method,with 31 patients in each group.The control group was given conventional medical treatment,and the experimental group was treated with Yishen Huayu Jiangzhuo Recipe on this basis.Efficacy,glucose metabolism,kidney function,vascular endothelial functionand autophagy related proteins were compared between the two groups.Results After treatment,the total effective rate in the experimental group was higher than that in the control group(P<0.05).The levels of fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 h PG),glycated hemoglobin(HbA1c),serum creatinine(SCr),blood urea nitrogen(BUN)and 24 h urine total protein(U-TP)in the two groups decreased,and the levels in the experimental group were lower than those in the control group(P<0.05).The level of nitric oxide(NO)in the experimental group was higher than that in the control group,and the levels of endothelin 1(ET-1)and von Willebrand factor(vWF)were lower than those in the control group(P<0.05).The levels of Beclin1 and ATG7 in the two groups increased,and the levels in the experimental group were higher than those in the control group(P<0.05).Conclusion The combination of Yishen Huayu Jiangzhuo Recipeand conventional western medicine can significantly improve the efficacy in the treatment of patients with DN,lower the blood glucose levels,increase the autophagy related protein expressions and improve the vascular endothelial function.
