中药封包疗法联合整脊推拿治疗神经根型颈椎病临床疗效研究

摘　　要：目的探讨中药封包疗法联合整脊推拿治疗神经根型颈椎病(cervical radicular spondylosis,CRS)的效果及对颈椎活动度、疼痛情况的影响。方法选取洪湖市人民医院2020年1月—2023年5月收治的CRS患者患者128例,按照均等随机单盲法分为对照组和试验组,各64例。两组均接受常规治疗,对照组在常规治疗基础上采用整脊推拿法进行治疗,试验组在对照组基础上联合中药封包疗法治疗,对比两组治疗前后中医证候积分、颈椎功能障碍指数[颈椎功能障碍指数量表(neck dysfunction index,NDI)]、疼痛程度[疼痛视觉模拟评分(visual analogue scale,VAS)]、颈椎活动度与生理曲度及生活质量[生活质量评分表(quality of life questionnaire-core 30,QOL-30)]。结果治疗前两组各中医证候积分、NDI、VAS、QOL-30评分、颈椎活动度及生理曲度比较差异均无统计学意义(P>0.05);治疗后,两组各中医证候积分、NDI及VAS均显著降低,且试验组低于对照组(P<0.05);治疗后,两组颈椎活动度、生理曲度及QOL-30评分显著升高,且试验组显著高于对照组(P<0.05)。结论采用中药封包疗法联合整脊推拿治疗CRS,可有效改善临床症状,缓解颈部疼痛,促进颈椎活动度及生理曲度恢复,对提升患者生活质量有利。Objective To explore the effect of traditional Chinese medicine packet therapy combined with chiropractic Tuina in the treatment of cervical radicular spondylosis(CRS)and its influence on cervical range of motion and pain.Methods A total of 128 patients with CRS admitted to the hospital from January 2020 to May 2023 were selected as divided into control group and experimental group according to equal random single blind method,with 64 cases in each group.Both groups received conventional treatment,and the control group was treated with chiropractic Tuina on the basis of conventional treatment,and the experimental group was given traditional Chinese medicine packet therapy on the basis of the control group.TCM syndromes scores,cervical dysfunction index[neck dysfunction index(NDI)],pain degree[visual analogue scale(VAS)],cervical range of motion and physiological curvature,and quality of life[quality of life questionnaire-core 30(QOL-30)]were compared between the two groups before and after treatment.Results Before treatment,there were no statistical differences in the TCM syndromes scores,NDI score,VAS score,QOL-30 score,cervical range of motion or physiological curvature between the two groups(P>0.05).After treatment,the TCM syndromes scores,NDI score and VAS score were significantly decreased in both groups,and the above scores in the experimental group were lower than those in the control group(P<0.05).The cervical range of motion,physiological curvature and QOL-30 score in both groups after treatment were significantly increased,and the above indicators in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion Traditional Chinese medicine packet therapy combined with chiropractic Tuina in the treatment of CRS can effectively improve the clinical symptoms,relieve the neck pain,and promote the cervical range of motion and physiological curvature recovery,and it is beneficial to improving the quality of life of patients.
