双镜联合胆总管切开取石一期缝合在基层医院的应用

摘　　要：目的:探讨双镜联合(腹腔镜联合胆道镜)胆总管一期缝合与T管引流治疗慢诊胆总管结石在基层医院的应用效果。方法:回顾性分析64例双镜联合胆总管探查治疗慢诊胆总管结石的患者资料,按照手术方法不同分为观察组(32例)和对照组(32例)。观察组患者采用一期缝合治疗,对照组采用T管引流治疗,分析比较两组患者手术时间、术中出血量、肛门通气时间、平均住院费用、住院时间、并发症发生率、生命质量。结果:两组患者术中出血量比较,差异无统计学意义(P>0.05);观察组患者手术时间、肛门通气时间、住院时间、住院费用均低于对照组(P<0.05);观察组患者并发症发生率低于对照组(P<0.05);观察组患者生存质量评分高于对照组(P<0.05)。结论:在基层医院双镜联合胆总管一期缝合治疗慢诊胆总管结石患者安全、有效、可行,不仅手术时间短,患者恢复快,节省费用,而且还能降低患者术后并发症发生率,提高患者生命质量。Objective:To explore the comparative effect of dual-scope combined(laparoscopy combined with choledochoscopy)one-stage choledochal suture and T-tube drainage for the treatment of chronic choledocholithiasis in primary hospitals.Method:The data of 64 patients with dual-scope combined choledochotomy for the treatment of chronic choledochal stones were retrospectively analysed.They were randomly divided into two groups,which were divided into observation group(32 cases)and control group(32 cases)according to different surgical methods.The observation group was treated with one-stage suture and the control group was treated with T-tube drainage.The analysis compares the operation time,intraoperative bleeding,anal ventilation time,average hospital costs,hospital stay,complication rate,and quality of life of patients in the two groups.Results:The difference in intraoperative bleeding between the two groups was not statistically significant(P>0.05);the observation group was lower than the control group in terms of operation time,anal ventilation time,hospitalisation time,and hospitalisation cost(P<0.05).The complication rate of patients in the observation group was low(P<0.05).The quality of survival score was high in the observation group(P<0.05).Conclusion:In primary hospitals,double mirror combined with one-stage suture of common bile duct is safe,effective and feasible for the treatment of patients with chronic choledochal stones,which not only has a short operation time,fast recovery of patients and saves costs,but also reduces the incidence of postoperative complications and improves the quality of life of patients.
