摘 要:目的:探究微柱凝胶技术在新生儿血型不合溶血病诊断中的应用价值。方法:选择医院收治的100例ABO血型不合溶血病患儿作为研究对象,采用随机数表法分为微柱凝胶法(MCGT)组和试管法(MPT)组,各50例患儿。对照组采用试管法(MPT),观察组采用微柱凝胶法(MCGT)。对比两组患儿血型检出率及MPT、MCGT在新生儿血型不合溶血病中的诊断价值。结果:所有参检的母体血型均为O型,ABO血型不溶血病患儿中检出56例A型血,44例B型血,分别占56.00%、44.00%。MCGT组患者交叉配血成功率高于MPT组,其交叉配血不合率低于MPT组,差异有统计学意义(P<0.05)。该研究共行交叉配血200次,ROC曲线显示,MCGT在交叉配血应用中准确度较高,其中放散液MCGT的AUC为0.825,灵敏度为1.000,特异度为0.650;其次为血清MCGT的AUC为0.756,灵敏度为1.000,特异度为0.512。结论:MCGT技术在新生儿血型不合溶血病诊断中具有较高的临床应用价值,有助于新生儿输血方案的制定,值得推广应用。Objective:To investigate the application value of microcolumn gel technology in the diagnosis of hemolytic disease with blood group incompatibility in newborns.Methods:100 cases of children with ABO blood group incompatibility hemolytic disease admitted to the hospital were selected as the study subjects,and were divided into two groups of 50 cases each by using the random number table method.The test tube method(MPT)was used in the control group and the microcolumn gel method(MCGT)was used in the observation group.The blood group detection rate and the diagnostic value of MPT and MCGT in neonatal blood group incompatibility hemolytic disease were compared between the two groups.Results:All the maternal blood types were O,56 cases of ABO blood type A blood,44 cases of B blood,accounting for 56.00%and 44.00%,respectively.In this study,a total of 200 times of cross-matching blood were performed.ROC curve showed that MCGT had high accuracy in cross-matching blood application,with AUC of 0.825,sensitivity of 1.000 and specificity of 0.650 for the released solution MCGT.The AUC of serum MCGT was 0.756,the sensitivity was 1.000,and the specificity was 0.512.Conclusion:MCGT technology has high clinical application value in the diagnosis of hemolytic disease of neonatal blood group incompatibility,which helps the development of neonatal blood transfusion program,and is worth popularizing and applying.
