摘　　要：目的:探讨慢性阻塞性肺疾病(COPD)合并慢性呼吸衰竭出院患者延续性护理需求现状及其影响因素。方法:采用便利抽样法,选取某医院收治的200例COPD合并慢性呼吸衰竭患者,使用自行设计的调查问卷,统计分析其延续性护理需求状况,并对延续性护理需求的影响因素进行分析。结果:200例患者延续性护理需求总得分为(49.12±1.65)分,其中各维度得分由高到低分别为疾病知识需求(19.21±1.02)分、生活指导需求(12.03±0.72)分、社会支持需求(11.54±0.76)分及心理需求(6.36±0.56)分;需求排名前三的条目分别为肺康复锻炼、家庭氧疗指导及疾病相关症状的自我观察及紧急处理措施;月收入、年龄、受教育程度、mMRC分级是COPD合并慢性呼吸衰竭出院患者延续性护理需求的独立影响因素(P<0.05)。结论:COPD合并慢性呼吸衰竭出院患者延续性护理需求较为强烈,月收入、年龄、受教育程度、mMRC分级是影响延续性护理需求的相关因素,应针对性提供延续性护理服务,促进患者生活质量的提高。Objective:To analyze the current situation and influencing factors of continuing nursing needs of discharged patients with chronic obstructive pulmonary disease(COPD)and chronic respiratory failure.Methods:Two hundred patients with COPD and chronic respiratory failure who were admitted to the hospital were selected by convenience sampling.A survey on their needs for continuing nursing was conducted using self-designed questionnaires,and the influencing factors of continuing nursing needs were analyzed.Results:The total score of continuing nursing needs of the 200 patients was(49.11±1.65).The scores for different dimensions listed from high to low was as follow:need for disease knowledge(19.21±1.02),need for life guidance(12.03±0.72),need for social support(11.54±0.76),and psychological need(6.36±0.56).The top three items of the needs were pulmonary rehabilitation exercise,home oxygen therapy guidance,and self observation and emergency response measures for disease related symptoms.Monthly income,age,education level,and mMRC grade were independent influencing factors of continuing nursing needs of discharged patients with COPD and chronic respiratory failure(P<0.05).Conclusion:Patients with COPD and chronic respiratory failure who are discharged from the hospital have strong needs for continuing nursing.Monthly income,age,education level,and mMRC grade are related factors that influence the needs for continuing nursing.Targeted continuing nursing services should be provided to promote the improvement of patients'life quality.
