摘　　要：目的 探讨手术时间对老年患者术后发生下肢深静脉血栓(DVT)的影响。方法 收集2021年4月至2023年10月于首都医科大学附属北京积水潭医院进行手术的280例患者的临床资料,按照中位手术时间的不同将其分为短时组(n=140,<159 min)和长时组(n=140,>159 min)。比较两组患者手术相关指标以及住院期间下肢DVT的发生率。结果 住院期间共25例患者发生下肢DVT,其中,症状性下肢DVT为14例,无症状下肢DVT为11例。不同性别、是否发生恶性肿瘤、是否进行骨科手术中下肢DVT发生率比较,差异均无统计学意义(P>0.05)。短时组患者下肢DVT发生率为5.0%,低于长时组患者的12.9%(P<0.05)。两组患者住院期间感染率、出血率比较,差异均无统计学意义(P>0.05)。结论 手术时间与老年手术患者术后发生下肢DVT风险有关,临床实践中应注意评估患者的发生风险,积极做好预防。Objective To investigate the effect of operation time on postoperative deep vein thrombosis(DVT)of lower extremity in elderly patients.Method Clinical data of 280 patients undergoing surgery in Beijing Jishuitan Hospital,Capital Medical University from April 2021 to October 2023 were collected and divided into short-term group(n=140,<159 min)and long-term group(n=140,>159 min)according to different median operation time.The incidence of DVT in lower extremity during hospitalization was compared between the two groups.Result A total of 25 patients developed DVT during hospitalization,including 14 patients with symptomatic DVT and 11 patients with asymptomatic DVT.Comparison of the incidence of DVT in lower extremity by gender,whether malignant tumor occurred,and whether orthopedic surgery was performed,the differences were not statistically significant(P>0.05).The incidence of DVT was 5.0%in the short period group,lower than 12.9%in the long period group(P<0.05).There were no significant differences in infection rate and bleeding rate between two groups during hospitalization(P>0.05).Conclusion Operation time is related to the risk of postoperative DVT in the lower extremity of elderly surgery patients.In clinical practice,attention should be paid to assessing the risk of patients and actively taking preventive measures.
