超声造影联合血清白细胞介素-6、白细胞介素-9水平对颈动脉大动脉炎活动性的诊断价值

摘　　要：目的 探讨超声造影(CEUS)联合血清白细胞介素(IL)-6、IL-9水平对颈动脉大动脉炎活动性的诊断价值。方法 收集2019年2月至2021年10月于新疆维吾尔自治区人民医院就诊的128例颈动脉大动脉炎患者的临床资料,根据印度Takayasu临床活动评分(ITAS2010)将其分为活动期组(n=56,ITAS2010评分≥5分)和非活动期组(n=72,ITAS2010评分<5分)。比较两组患者CEUS参数及血清白细胞介素(IL)-6、IL-9水平,绘制受试者工作特征(ROC)曲线分析CEUS联合血清IL-6、IL-9水平对颈动脉大动脉炎患者活动性的诊断价值。结果 活动期组患者血管化分级(即颈总动脉壁新生血管化分级)≥2级比例、增强程度(EI)、峰值强度(DPI)均高于非活动期组患者,达峰时间(TTP)低于非活动期组患者,差异均有统计学意义(P<0.05)。活动期组患者血清IL-6、IL-9水平均高于非活动期组患者,差异均有统计学意义(P<0.05)。CEUS参数(血管化分级、EI、DPI、TTP)联合诊断颈动脉大动脉炎活动性的灵敏度、曲线下面积(AUC)均高于各参数单独诊断。CEUS参数联合血清IL-6、IL-9水平诊断颈动脉大动脉炎活动性的灵敏度、AUC均高于各自单独诊断。结论 CEUS参数联合血清IL-6、IL-9水平对颈动脉大动脉炎患者活动性具有一定的诊断价值,但联合诊断效能更高。Objective To investigate the diagnostic value of contrast-enhanced ultrasound(CEUS)combined with serum interleukin(IL)-6 and IL-9 levels in the takayasu arteritis activity of carotid artery.Method Clinical data of 128 patients with carotid arteritis admitted to the Xinjiang Uygur Autonomous Region People's Hospital from February 2019 to October 2021 were collected and divided into active group[n=56,Indian Takayasu clinical activity score(ITAS2010)≥5 points]and inactive group(n=72,ITAS2010 score<5 points)according to ITAS2010 score.CEUS parameters and serum interleukin-6 and IL-9 levels were compared between the two groups,the receiver operator characteristic(ROC)curve was drawn to analyze the diagnostic value of CEUS combined with serum IL-6 and IL-9 levels in the activity of patients with carotid arteritis.Result The proportion of vascularization grade(common carotid artery wall neovascularization grade)≥2,enhanced intensity(EI)and derived peak intensity(DPI)in the active group were higher than those in the inactive group,and the time to peak(TTP)was lower than those in the inactive group,with statistical significance(P<0.05).The levels of serum IL-6 and IL-9 in active group were higher than those in non-active group,the differences were statistically significant(P<0.05).The sensitivity and area under the curve(AUC)of CEUS parameters(vascularization grade,EI,DPI,TTP)combined with the diagnosis of carotid arteritis activity were higher than those of each parameter alone.The sensitivity and AUC of CEUS parameters combined with serum IL-6 and IL-9 levels in the diagnosis of carotid arteritis activity were higher than those separately diagnosed.Conclusion CEUS and the levels of serum IL-6 and IL-9 all have a certain diagnostic value in patients with takayasu arteritis activity of carotid artery,but the combined diagnostic efficacy is higher.
