断指再植患者围手术期血管危象危险因素分析

摘　　要：目的 探讨断指再植患者围手术期发生血管危象的危险因素。方法 收集2021年3月至2024年5月中国人民解放军海军第九七一医院收治的198例断指再植患者的临床资料,按照围手术期是否发生血管危象将患者分为危象组(n=41)和对照组(n=157)。收集所有患者入院时的临床资料,分析断指再植患者围手术期发生血管危象的危险因素。结果 危象组患者中有吸烟史、受伤至手术的时间超过10 h、血管移植和末节离断的比例均高于对照组患者,差异均有统计学意义(P<0.05)。多因素分析结果显示,有吸烟史、受伤至手术时间超过10 h、有血管移植以及末节离断均是断指再植患者围手术期发生血管危象的独立危险因素(P<0.05)。结论 吸烟、受伤至手术的时间超过10 h、有血管移植以及末节离断均可增加断指再植患者围手术期发生血管危象的风险,对此类患者应提前做好沟通和血管危象的预防。Objective To investigate the risk factors for perioperative vascular crisis in patients undergoing finger replantation.Method The clinical data of 198 patients who underwent finger replantation admitted to the No.971 Hospital of the People's Liberation Army Navy from March 2021 to May 2024 were collected.Patients were divided into crisis group(n=41)and control group(n=157)based on whether vascular crisis occurred during the perioperative period.The clinical data of all patients upon admission were collected,the risk factors for vascular crisis in patients undergoing finger replantation during the perioperative period was analyzed.Result The proportion of patients with history of smoking,injury to surgery time exceeding 10 h,presence of transplanted veins,and distal amputation in the crisis group were higher than those in the control group,and the difference were statistically significant(P<0.05).The results of multiple factor analysis showed that smoking history,injury to surgery time exceeding 10 h,presence of transplanted veins,and distal amputation were independent risk factors for perioperative vascular crisis in patients undergoing finger replantation(P<0.05).Conclusion Smoking,injury to surgery time exceeding 10 h,presence of transplanted veins,and distal amputation can increase the risk of vascular crisis in patients undergoing finger replantation during the perioperative period.Communication and prevention of vascular crisis should be done in advance for such patients.
