摘　　要：目的 探讨下肢静脉曲张的临床与检验特点。方法 收集2021年8月至2023年7月于青海省中医院接受诊治的146例下肢静脉曲张患者的临床资料作为病例组,并且按照年龄、性别进行配对,纳入同期进行健康体检且无明显疾病的146例健康人群作为健康对照组。收集所有患者诊断下肢静脉曲张时的临床资料,包括基本信息、实验室检测结果、下肢静脉曲张病情及治疗情况,分析并总结下肢静脉曲张的临床与检验特点。比较两组研究对象的临床特征,分析下肢静脉曲张的临床特点。结果 病例组中体力劳动者、吸烟、存在红细胞增多症的比例及血红蛋白水平均高于健康对照组,凝血酶原时间和活化部分凝血酶原时间均长于健康对照组,差异均有统计学意义(P<0.05)。146例下肢静脉曲张患者的临床表现-病因学-解剖学-病理生理学(CEAP)分级为1~6级,其中,1~2级29例,3~4级73例,5级35例,6级9例。合并血栓性静脉炎31例。109例患者接受手术治疗,均获得临床治愈。术后住院期间共15例患者发生手术相关并发症,其中,发生下肢深静脉血栓患者4例(均未合并红细胞增多症),出现疼痛感的患者11例,均得到缓解。结论 下肢静脉曲张易合并血栓性静脉炎、红细胞增多症,部分患者需多次住院接受手术治疗。Objective To investigate the clinical and laboratory characteristics of lower extremity varicose veins.Method The clinical data of 146 patients with lower extremity varicose veins who received diagnosis and treatment at Qinghai Province Traditional Chinese Medicine Hospital from August 2021 to July 2023 were collected as the case group,and match them according to age and gender to include 146 healthy individuals who underwent physical examinations during the same period and had no obvious diseases as the healthy control group.The clinical data of all patients diagnosed with lower extremity varicose veins were collected,including basic information,laboratory test results,condition and treatment of lower extremity varicose veins,and the clinical and laboratory characteristics of lower extremity varicose veins in high-altitude areas were analyzed and summarized.Result The proportion of physical laborers,smokers,and patients with polycythemia and hemoglobin levels in the case group were higher than those in the healthy control group,the prothrombin time and activated partial thromboplastin time were both longer than those in the healthy control group,and the differences were statistically significant(P<0.05).The clinical etiology anatomy pathophysiology(CEAP)grading of 146 patients with lower extremity varicose veins was 1-6 grades,including 29 cases of grade 1-2,73 cases of grade 3-4,35 cases of grade 5,and 9 cases of grade 6.A total of 31 cases of combined thrombophlebitis.A total of 109 patients underwent surgical treatment and all achieved clinical cure.During the postoperative hospitalization period,a total of 15 patients experienced surgical related complications,including 4 patients with lower extremity deep vein thrombosis(none of whom had concomitant polycythemia)and 11 patients who experienced pain,all of whom were relieved.Conclusion Lower extremity varicose veins are prone to complications such as thrombophlebitis and polycythemia,and some patients require multiple hospitalizations for surgical treatment.
