一氧化氮联合西地那非治疗新生儿持续性肺动脉高压的效果分析

摘  要:目的:分析一氧化氮(NO)联合西地那非治疗新生儿持续性肺动脉高压(PPHN)的效果。方法:将2019年1月—2023年12月贵阳市妇幼保健院收治的200例PPHN患儿随机分为对照组(100例,给予NO吸入治疗)和观察组(100例,在对照组基础上给予西地那非治疗)。比较两组治疗效果。结果:治疗后,两组血气指标优于治疗前,且观察组优于对照组(P<0.05)。两组内皮素-1水平、缺氧诱导因子-1α水平、肺动脉收缩压降低,且观察组低于对照组(P<0.05);两组舒张早期心室充盈速度最大值/舒张晚期心室充盈速度最大值、短轴缩短率、右室射血分数、左室射血分数均高于治疗前,且观察组高于对照组(P<0.05)。结论:NO联合西地那非治疗新生儿PPHN的效果良好,可改善患儿血气指标、血清学指标及心功能。Objective:To analyze the effect of nitric oxide(NO)combined with sildenafil in the treatment of persistent pulmonary hypertension of the newborn(PPHN).Methods:A total of 200 children with PPHN admitted to Guiyang Maternal and Child Health Hospital from January 2019 to December 2023 were randomly divided into control group(100 cases,given NO inhalation therapy)and observation group(100 cases,treated with sildenafil on the basis of the control group).The treatment effects were compared between the two groups.Results:After treatment,the blood gas indexes of the two groups were better than those before treatment,and the observation group was better than the control group(P<0.05).The levels of endothelin-1,hypoxia-inducible factor-1αand pulmonary artery systolic blood pressure in the observation group were lower in the observation group than those in the control group(P<0.05),and the maximum ventricular filling velocity in early diastole and late diastole,the short-axis shortening rate,right ventricular ejection fraction and left ventricular ejection fraction in the two groups were higher than those before treatment,and the observation group was higher than that in the control group(P<0.05).Conclusions:NO combined with sildenafil has a good effect in the treatment of neonatal PPHN,and can improve blood gas indexes,serological indexes and cardiac function in children.
