关节镜下松解术联合肩袖修复术治疗肩袖损伤合并肩关节周围炎的效果分析

摘　　要：目的:分析关节镜下松解术联合肩袖修复术治疗肩袖损伤合并肩关节周围炎的效果。方法:选取2019年1月—2023年12月东南大学附属中大医院江北院区收治的肩袖损伤合并肩关节周围炎患者100例作为研究对象,以随机数字表法分为对照组和试验组,各50例。对照组实施手法松解联合关节镜肩袖修复术,试验组实施关节镜下松解术联合肩袖修复术。比较两组手术指标、肩关节活动度、患侧肌肉力量、疼痛程度。结果:试验组手术时间、住院时间短于对照组,术中出血量少于对照组(P<0.001)。术后4个月,两组外展、前屈、内旋、后伸、外旋评分均升高,且试验组高于对照组(P<0.05)。术后4个月,两组患侧肌肉力量增加,且试验组大于对照组(P<0.05);两组疼痛评分降低,且试验组低于对照组(P<0.05)。结论:关节镜下松解术联合肩袖修复术治疗肩袖损伤合并肩关节周围炎的治疗效果较好,可改善手术指标,减轻疼痛,促进肩关节活动度恢复,增加患侧肌肉力量。Objective:To analyze the effect of arthroscopic release combined with rotator cuff repair in the treatment of rotator cuff injury combined with shoulder periarthritis.Methods:A total of 100 patients with rotator cuff injury and shoulder periarthritis who received treatment at the Jiangbei Branch of Zhongda Hospital of Southeast University from January 2019 to December 2023 were selected as the study subjects.They were divided into control group and experimental group according to random number table method,with 50 patients in each group.The control group received release maneuver and arthroscopic rotator cuff repair,while the experimental group received arthroscopic release and rotator cuff repair.The surgical indicator,range of motion of shoulder joint,muscle strength of the affected side and pain degree were compared between the two groups.Results:The operation time and the length of hospital stay in the experimental group were shorter than those in the control group,and the intraoperative blood loss in the experimental group was less than that in the control group(P<0.001).At 4 months after surgery,the scores of abduction,anterior flexion,internal rotation,posterior extension and external rotation in two groups increased,and the scores in the experimental group were higher than those in the control group(P<0.05).At 4 months after surgery,the muscle strength of the affected side in two groups increased,and this parameter in the experimental group was larger than that in the control group(P<0.05).The pain scores in two groups decreased,and the scores in the experimental group were lower than those in the control group(P<0.05).Conclusion:Arthroscopic release combined with rotator cuff repair has a good therapeutic effect in the treatment of rotator cuff injury complicated with shoulder periarthritis,and can improve surgical indicators,relieve pain,promote the recovery of shoulder joint range of motion,and increase the muscle strength of the affected side.
