摘 要:目的:探讨神经传导速度检测在糖尿病周围神经病变诊断中的应用价值。方法:选取贵州省遵义市第一人民医院(遵义医科大学第三附属医院)2022年5月—2024年3月收治的98例疑似糖尿病周围神经病变患者作为研究对象,均进行神经传导速度检测和高频超声检查。以病理检查结果作为“金标准”,分析神经传导速度检测和高频超声检查诊断糖尿病周围神经病变的效能;根据病理检查结果将患者分为非病变组和病变组,比较两组正中神经、尺神经、腓总神经运动神经传导速度(MCV)、感觉神经传导速度(SCV)。结果:98例疑似糖尿病周围神经病变患者经病理检查确诊为阳性67例,神经传导速度检测诊断为阳性66例,高频超声检查诊断为阳性65例。神经传导速度检测的特异度、灵敏度、准确度高于高频超声检查(P<0.05)。病变组正中神经、尺神经、腓总神经MCV、SCV均低于非病变组(P<0.05)。结论:神经传导速度检测在糖尿病周围神经病变诊断中的应用价值较高,可以作为评估糖尿病患者神经功能早期病变的参考依据。Objective:To investigate the application value of nerve conduction velocity test in the diagnosis of diabetic peripheral neuropathy.Methods:A total of 98 suspected diabetic peripheral neuropathy patients admitted to the First People's Hospital of Zunyi in Guizhou Province(The Third Affiliated Hospital of Zunyi Medical University)from May 2022 to March 2024 were selected as the study subjects.All patients underwent nerve conduction velocity test and high-frequency ultrasound examination.The pathological examination results were used as the"gold standard".The efficiency of nerve conduction velocity test and highfrequency ultrasound examination for diagnosing diabetic peripheral neuropathy was analyzed.According to the pathological examination results,the patients were divided into non-lesion group and lesion group.The motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)of median nerve,ulnar nerve and common peroneal nerve were compared between the two groups.Results:Among 98 patients with suspected diabetic peripheral neuropathy,67 cases were diagnosed positive by pathological examination,66 cases were diagnosed positive by nerve conduction velocity test,and 65 cases were diagnosed positive by high-frequency ultrasound examination.The specificity,sensitivity and accuracy of nerve conduction velocity test were higher than those of high-frequency ultrasound examination(P<0.05).The MCV and SCV of median nerve,ulnar nerve and common peroneal nerve in the lesion group were lower than those in the non-lesion group(P<0.05).Conclusion:Nerve conduction velocity test has high application value in the diagnosis of diabetic peripheral neuropathy,and can be used as a reference for evaluating the early lesion of neurological function of diabetic patients.
