县域医院构建多种医联体模式实践探索——以佛山市三水区人民医院为例

摘　　要：为贯彻国家医联体建设发展要求,佛山市三水区人民医院依照区域特点积极部署,建立医联体,探索构建县域医院两级多种医联体模式。“对上”,主动和省、市大医院对接,通过构建省—市—县区级医院医联体,借力引力发展。“对下”,按照合作需求和意愿分别建设县区—镇街级紧密型医联体和松散型医联体。紧密型医联体建设在医联体框架下,以专科联盟为支撑,管理团队和技术团队进驻,辅以非常驻专家到院指导和远程会诊;松散型医联体以专家定期或不定期查房、授课、技术指导为主。通过双向转诊、专科建设、满意度、医院管理等多维度比较,多种医联体模式互补让优质医疗资源下沉基层,促进医疗资源上下贯通,提高医疗服务体系整体效能,能够更好地实施分级诊疗制度,满足群众就近就医健康需求,提升患者就医体验感,促进患者满意度提升。To comply with the national requirements for the construction and development of medical alliances,the Sanshui District Peoples Hospital of Foshan,has actively deployed according to the regional characteristics,establishing a medical alliance and exploring the construction of a two-level multi-type medical alliance system for county-level hospitals.“Upwards”,the hospital actively connects with major hospitals at the provincial and municipal levels,and builds a provincial-municipal-county-level hospital medical alliance to leverage the gravitational development.“Downwards”,the hospital builds a county-town-level tightly-knit medical alliance and a loose medical alliance according to the needs and willingness for cooperation.The tightly-knit medical alliance is built within the framework of the medical alliance,with specialty alliances as the support,management teams and technical teams moving in,and supplemented by non-resident experts coming to the hospital for guidance and remote consultation.The loose medical alliance is mainly based on regular and irregular on-site visits,lectures,and technical guidance by experts.Through comparisons in multiple dimensions such as two-way referrals,specialty construction,satisfaction,and hospital management,the hospital explores how to complement the various medical alliance models to facilitate the decentralization of high-quality medical resources,promote the smooth flow of medical resources up and down,improve the overall efficiency of the medical service system,better implement the principle of tiered medical care,meet the healthcare needs of local residents closer to home,enhance patient experience to promote satisfaction.
