重庆口岸出境留学生两种结核病筛查皮肤试验结果对比

摘  要:目的探讨重组结核杆菌融合蛋白(EC)和结核菌素纯蛋白衍生物(TB-PPD)皮肤试验在出境留学生结核病筛查中的作用,为口岸结核病筛查策略提供依据。方法2021年5月—2022年4月,对重庆口岸出境留学生进行EC和TB-PPD同体双臂联合试验,48~72 h后测量注射部位红晕和硬结的平均直径,分析两个皮试之间的差异。结果共纳入留学生108人,TB-PPD试验阳性率(41.67%)明显高于EC(3.70%),差异有统计学意义(χ^(2)=6.39,P<0.001),不同性别、年龄留学生的EC试验阳性率差异无统计学意义(χ^(2)=0.00,P=0.68;χ^(2)=3.83,P=0.43),不同性别、年龄人员的TB-PPD阳性率差异无统计学意义(χ^(2)=0.18,P=0.67;χ^(2)=2.73,P=0.60);两种方法的阴性率、一般阳性率、强阳性率差异均有统计学意义(P<0.001),两种方法的结果一致性较低(Kappa=0.10,P=0.02)。有卡介苗接种史者TB-PPD试验阳性率(39.56%)高于EC(4.40%),差异有统计学意义(χ^(2)=32.81,P<0.001)。结论EC皮肤试验结果受卡介苗接种影响小,适用于大规模人群及口岸结核筛查。Objective To discuss the effects of recombinant Mycobaerium tuberculosis fusion protein(EC)test and the pure protein derivative of tuberculin(TB-PPD)skin test in tuberculosis screening among overseas students,to provide a basis for tuberculosis screening strategies at ports.Methods From May 2021 to April 2022,EC and TB-PPD two-arm skin tests were conducted on outbound international students at Chongqing port.The average diameter of redness and induration at the injec-tion site was measured in 48 to 72 hours,and difference between the two skin tests was analyzed.Results A total of108 international students were included.The positive rate of TB-PPD test(41.67%)was significantly higher than that of EC(3.70%),the difference was statistically significant(χ^(2)=6.39,P<0.001).However,no statistically significant differences was observed in the positive rates of EC test across different gender and age international students(χ^(2)=0.00,P=0.68;χ^(2)=3.83,P=0.43).Similarly,there was no significant differences in the positive rates of TB-PPD when stratified by gender and age(χ^(2)=0.18,P=0.67;χ^(2)=2.73,P=0.60).There were statistically significant differences in negative rates as well as general and strong positive rates between the two methods(P<0.001),indicated a low possibility of homogeneities between the two methods(Kappa=0.10,P=0.02).Furthermore,the positive rate of TB-PPD test in the students with BCG vaccination was found to be 39.56%,which was higher than that of EC(4.40%),with statistically significant differences(χ^(2)=32.81,P<0.001).Conclusion EC skin test was less influenced by BCG vaccine,making it suitable for tuberculosis screening in large population and at ports.
