围手术期健康教育结合基于冰山理论的针对性护理在乳腺癌患者中的应用

摘　　要：目的分析围手术期健康教育结合基于冰山理论的针对性护理在乳腺癌患者中的应用价值。方法选取徐州医科大学附属医院2020年8月至2022年8月收治的84例乳腺癌患者,以随机数字表法分别纳入观察组(42例)和对照组(42例),均行围手术期综合护理及健康教育,观察组加用基于冰山理论的针对性护理。评估2组患者入院时、出院前的自护能力、自尊状态和心理状态变化,并比较2组的术后并发症发生率。结果2组在出院前的自护能力量表(ESCA)各维度评分及总分、状态自尊量表(SSES)各维度评分及总分均较入院时升高,焦虑自评量表(SAS)和抑郁自评量表(SDS)评分均较入院时下降,且观察组均更优于对照组,差异均有统计学意义(P均<0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。结论在围手术期健康教育的基础上应用基于冰山理论的针对性护理,有助于乳腺癌患者自护能力、自尊水平的提高,以及心理状态的改善,且能够降低患者术后并发症的发生风险。Objective To analyze the application value of perioperative health education combined with targeted nursing based on iceberg theory in patients with breast cancer.Methods A total of 84 patients with breast cancer admitted to the Affiliated Hospital of Xuzhou Medical University from August 2020 to August 2022 were selected and included in the observation group(42 cases)and the control group(42 cases)respectively with the random number table method.All patients received perioperative comprehensive nursing and health education.The observation group additionally used targeted nursing based on iceberg theory.The changes of self-care ability,self-esteem state,and mental status at admission and before discharge were evaluated,and the incidence of postoperative complications between the two groups were compared.Results The scores and total scores of each dimension of the Exercise of Self-Care Agency(ESCA)and the State Self-Esteem Scale(SSES)of both groups before discharge increased compared to admission,while the scores of the Self-Rating Anxiety Scale(SAS)and the Self-Rating Depression Scale(SDS)decreased compared to admission.Moreover,the above scores of the observation group were better than those of the control group,and the differences were statistically significant(all P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion On the basis of perioperative health education,the application of targeted nursing based on iceberg theory is helpful to improve the self-care ability,self-esteem level and mental status of breast cancer patients,and reduce the risk of postoperative complications.
