心理弹性支持结合以头眼运动为主的感觉运动康复训练在脑卒中偏瘫患者中的应用

摘　　要：目的观察心理弹性支持结合以头眼运动为主的感觉运动康复训练在脑卒中偏瘫患者中的应用效果。方法选取2021年4月至2023年4月于海军军医大学第二附属医院(上海长征医院)就诊的脑卒中偏瘫患者共计106例,使用随机数字表法将其分配至观察组(53例)和对照组(53例)。2组均接受康复干预和心理弹性支持,观察组加用以头眼运动为主的感觉运动康复训练。分别于干预前和干预1个月、3个月后,使用Brunnstrom评定量表、日常生活能力量表(ADL)、功能性步态评估表(FGA)、感觉统合测试(SOT)评价2组患者的肢体功能、自理能力、步行能力及平衡功能变化。结果干预1个月和3个月后,观察组的上肢与下肢功能评分、ADL评分、FGA评分及SOT评分均较同一时间对照组更高(P均<0.05),且2组在上述时间点的评分与干预前比较均更高(P均<0.05)。结论在接受康复干预和心理弹性支持的同时积极开展以头眼运动为主的感觉运动康复训练,有助于促进脑卒中偏瘫患者肢体功能、自理能力、步行能力及平衡功能的恢复与改善。Objective To observe the application effect of psychological resilience support combined with sensory and motor rehabilitation training focused on head and eye movement in stroke patients with hemiplegia.Methods A total of 106 stroke patients with hemiplegia who visited the Second Affiliated Hospital of Naval Medical University(Shanghai Changzheng Hospital)from April 2021 to April 2023 were selected and randomly assigned to an observation group(53 cases)and a control group(53 cases)using a random number table method.Both groups received rehabilitation intervention and psychological resilience support,while the observation group received sensory and motor rehabilitation training focused on head and eye movement.The Brunnstrom Scale,Activities of Daily Living Scale(ADL),Functional Gait Assessment(FGA),and Sensory Organization Test(SOT)were used to evaluate the changes in limb function,self-care ability,walking ability,and balance function of the two groups of patients before and after 1 and 3 months of intervention,respectively.Results After 1 and 3 months of intervention,the upper and lower limb function scores,ADL score,FGA score,and SOT score of the observation group were higher than those of the control group at the same time(all P<0.05),and the scores of both groups were higher at the above time points compared with before intervention(all P<0.05).Conclusion While receiving rehabilitation intervention and psychological resilience support,actively carrying out sensory and motor rehabilitation training focused on head and eye movement can help promote the recovery and improvement of limb function,self-care ability,walking ability,and balance function of stroke patients with hemiplegia.
