摘 要:目的 系统评价三维电子内窥镜和传统二维内窥镜在胸腹腔手术中的临床安全及有效性。方法 计算机检索PubMed、EMBASE和CNKI数据库。收集近十年发表的关于三维电子内窥镜和二维内窥镜在胸腹腔应用的安全性及有效性文献。采用纽卡斯尔-渥太华量表(Newcastle-Ottawa Scale,NOS)量表对纳入的研究进行质量评价,借助RevMan5.4软件实施Meta分析。结果 最终纳入66篇文献,涵盖6867例患者。Meta分析结果表明,三维电子内窥镜可用于胸腹腔部位成像及手术操作。与传统二维内窥镜相比,三维电子内窥镜能显著缩短手术时间[MD=-12.70,95%CI(-17.13~-8.27)],减少出血量[MD=-10.87,95%CI(-5.48~-6.25)],缩短住院时间[MD=-0.39,95%CI(-0.55~-0.24)],并有效减少并发症发生率[OR=0.76,95%CI(0.66~0.87)]。结论 三维电子内窥镜融合了三维成像和电子内窥镜技术,可有效缩短胸腹腔镜手术时间、减少出血量和住院时间,降低术后并发症发生风险,为胸腹腔手术提供了更优的技术选择。Objective To evaluate the clinical safety and effecacy of 3D video endoscope and traditional 2D endoscope in thoracoabdominal surgery.Methods Relevant literatures were retrieved from databases,including PubMed,EMBASE and CNKI.The safety and efficacy of 3D video and 2D endoscopy in the thoracoabdominal cavity were collected.The Newcastle-Ottawa Scale(NOS)was used to evaluate the quality of the included studies,and meta-analysis was performed with RevMan 5.4 software.Results 66 articles were included,covering 6867 patients.The results of meta-analysis indicate that 3D video endoscope can be used for imaging and operation of thoracic and abdominal surgery.Compared with the traditional 2D endoscope,3D video endoscope can significantly shorten the operation time[MD=-12.70,95%CI(-17.13~-8.27)],reduce blood loss[MD=-10.87,95%CI(-5.48~-6.25)],and shorten the length of hospital stay[MD=-0.39,95%CI(--0.55~-0.24)].It also significantly reduced the incidence of complications[OR=0.76,95%CI(0.66-0.87)].Conclusion 3D video endoscopy,which combines 3D imaging and electronic endoscopy technology,can effectively shorten the time of thoracic laparoscopic surgery,reduce blood loss and hospital stay,reduce the risk of postoperative complications,and provide a better technical choice for thoracoabdominal surgery.
