早产儿早期体质量与支气管肺发育不良的关系

摘　　要：目的探讨胎龄<32周早产儿生后早期体质量与支气管肺发育不良(bronchopulmonary dysplasia,BPD)发生的关系。方法回顾性分析2016年12月—2021年12月厦门医学院附属第二医院收治的195例胎龄<32周早产儿的临床资料。其中BPD患儿46例作为BPD组;非BPD患儿149例,从中抽选出生胎龄、性别与BPD组相似的46例作为对照组。比较2组出生时、第1周、第2周、第3周、第4周体质量。结果2组出生体质量及第3、4周体质量比较,差异无统计学意义(P>0.05);BPD组第1、2周体质量分别为(1.17±0.04)kg、(1.29±0.04)kg,低于对照组的(1.28±0.03)kg、(1.41±0.03)kg,差异有统计学意义(P<0.05),r值分别为0.222、0.225。结论胎龄<32周早产儿出生后前2周的体质量与BPD发生呈低度线性相关,当出生体质量相似时,BPD组第1、2周体质量较低。Objective To Explore the relationship between early postnatal body mass and bronchopulmonary dysplasia(BPD)in premature infants with gestational age<32 weeks.Methods The clinical data of 195 premature infants with gestational age<32 weeks admitted to the Second Affiliated Hospital of Xiamen Medical College from December 2016 to December 2021 were retrospectively analyzed.Among them,46 children with BPD were included as the BPD group;149 cases of non BPD children were selected,and 46 cases with similar gestational age and gender to the BPD group were selected as the control group.The body mass of the two groups at birth,week 1,week 2,week 3 and week 4 was compared.Results There was no significant difference in birth weight,body weight at the 3rd and 4th week between the two groups(P>0.05).The body mass of the BPD group at week 1 and 2 was(1.17±0.04)kg and(1.29±0.04)kg,respectively,which was lower than those of the control group[(1.28±0.03)kg and(1.41±0.03)kg],and the differences were statistically significant(P<0.05),with r values of 0.222 and 0.225.Conclusion The body mass of premature infants with gestational age<32 weeks is weakly linearly correlated with the occurrence of BPD in the first 2 weeks after birth.When the birth body mass is similar,the BPD group has lower body mass in the first and second week s.
