摘 要:目的调查厦门地区献血员ABO血型血清学表型分布状况,丰富厦门地区ABO血型分布数据。方法采用微孔板法通过全自动血型分析仪对2018年1月—2022年12月302733名献血者样本进行血型正反定型和O细胞初筛,仪器初筛出现疑问的标本用试管盐水介质法进行正反定型复检确认并加做-H抗体,-AB抗体、-A1抗体和A2细胞及必要时进行吸收放散试验和谱细胞抗体筛查。比较各血型亚型检出率,每类亚型的具体分布情况以及不同性别在不同血型中的亚型检出情况及类孟买血型的检出情况。结果厦门地区献血员ABO血型表型特征分布为O>A>B>AB,ABO亚型的检出率为2.05/万;其中B型个体中检出的B亚型为5.92/万,B亚型中B_(X)和B_(3)占比最多;A型个体中检出的A亚型为0.80/万,检出的A亚型全部为男性;AB型个体中检出的AB亚型为6.07/万,AB亚型个体中男性的检出率为7.60/万,女性为3.02/万;各血型间亚型检出率比较差异有统计学意义(P<0.001),不同性别的各血型间的亚型检测率比较差异有统计学意义(P<0.001);类孟买血型阳性率为0.53/万(16/302733),16个类孟买血型1个为云南户籍,15个为福建户籍。结论厦门地区福建籍人口类孟买血型频率较高,B亚型和AB亚型比A亚型检出率高,A亚型和AB亚型中男性的比率比女性高,可根据每个地区的亚型分布特点建立相应的对应数量的稀有血型数据库。Objective To investigate the distribution of ABO blood group serological phenotype of blood donors in Xiamen area,enrich the distribution data of ABO blood group in Xiamen area.Methods The ABO forward grouping and reverse grouping and O cell screening were performed on 302733 blood donor samples by automatic blood group analyzer using microporous plate method.The samples with doubts in the preliminary screening of the instrument were rechecked for ABO forward grouping and reverse grouping by test tube saline medium method,antibody-H,antibody-AB,antibody-A1,and A2 cells were also tested,and absorption and release test and spectrum cell antibody screening were performed when necessary.The detection rate of different blood group subtypes,the specific distribution of each subtype,the detection of subtypes in different blood groups of different genders and the detection of quasi-Bombay blood group were compared.Results The phenotypic distribution of ABO blood group in Xiamen was O>A>B>A B,and the detection rate of ABO subtype was 2.05 per 10000.The detection rate of B subtype in type B individuals was 5.92 per 10000,and B_(X) and B_(3) accounted for the largest proportion of B subtype.The detection rate of A subtype in type A individuals was 0.80 per 10000,and all the detected A subtypes were male.The detection rate of AB subtype in type AB individuals was 6.07 per 10000,and the detection rate of AB subtype in male was 7.60 per 10000,and that in female was 3.02 per 10000,the difference in the detection rate of subtypes among different blood groups was statistically significant(P<0.001),and the difference in the detection rate of subtypes among different genders was statistically significant(P<0.001).The positive rate of para-Bombay blood group was 0.53 per 10000(16/302733),and 16 para-Bombay blood groups were 1 from Yunnan province and 15 from Fujian province.Conclusion The frequency of para-Bombay blood group in Fujian population in Xiamen area is higher,the positive rate of B subtype and AB subtype is higher than th
