摘  要:目的探讨基于Tecnobody系统的新型卒中后跌倒风险评估体系在临床应用中的效果。方法采用观察性研究方法,连续性纳入深圳市罗湖区人民医院康复医学科病区2022年9月—2023年11月的脑卒中住院患者236例。使用Tecnobody系统和传统Morse跌倒风险评估量表(Morse fall seale,MFS)同时对患者进行跌倒风险评估。记录2种评估工具的评估时长、风险分级结果、个性化预防措施以及患者实际住院期间跌倒发生情况,评价2种评估系统的准确性和应用满意度。结果Tecnobody系统评估时间为(7.3±2.8)min,短于MFS的(14.6±3.7)min,差异有统计学意义(P<0.05)。患者住院期间实际跌倒发生率为9.75%,Tecnobody系统评估的高危患者中13.68%发生跌倒,高于低危患者5.98%,差异有统计学意义(P<0.05)。Tecnobody系统诊断敏感度为69.2%,高于MFS的46.7%,差异有统计学意义(P<0.05)。两者特异度(65.8%vs.68.8%)、阳性预测值(23.1%vs.18.3%)和阴性预测值(93.2%vs.89.9%)比较,差异无统计学意义(P>0.05)。医护人员和患者对Tecnobody系统的总体满意度评分为(8.7±1.1)分、(9.1±0.8)分,高于MFS的(6.9±1.5)分、(7.3±1.2)分,差异均有统计学意义(P<0.001)。结论基于Tecnobody的新型卒中后跌倒风险评估体系具有较高的评估效率和诊断敏感度,操作简便,个性化干预措施合理可行。Objective To explore the effect of a new fall risk assessment system after stroke based on Tecnobody system in clinical application.Methods An observational study was conducted to continuously enroll 236 stroke inpatients from department of rehabilitation medicine in Luohu District People′s Hospital,Shenzhen from September 2022 to November 2023.Tecnobody system and the traditional Morse fall seale(MFS)were used to simultaneously assess the fall risk of patients.The evaluation time,risk classification results,personalized preventive measures and the actual falls of patients during hospitalization of the two assessment tools were recorded to evaluate the accuracy and application satisfaction of the two assessment systems.Results The assessment time for the Tecnobody system was(7.3±2.8)minutes,which was shorter than(14.6±3.7)minutes of the MFS,the difference was statistically significant(P<0.05).The actual fall incidence rate during hospitalization was 9.75%.Among patients assessed as high risk by the Tecnobody system,13.68%experienced falled,which was significantly higher than the 5.98%of low-risk patients,the difference was statistically significant(P<0.05).The diagnostic sensitivity of the Tecnobody system was 69.2%,which higher than 46.7%of the MFS,the difference was statistically significant(P<0.05).However,there were no statistically significant differences in specificity(65.8%vs.68.8%),positive predictive value(23.1%vs.18.3%),and negative predictive value(93.2%vs.89.9%)between the two systems(P>0.05).The overall satisfaction scores for the Tecnobody system from healthcare staff and patients are(8.7±1.1)points and(9.1±0.8)points respectively,which were higher than those for the MFS at(6.9±1.5)points and(7.3±1.2)points,the differences were statistically significant(P<0.001).Conclusion The novel fall risk assessment system after stroke based on Tecnobody has high assessment efficiency and diagnostic sensitivity.It is simple to operate and personalized intervention measures are reasonable and feasible.
