摘　　要：目的研究老年肿瘤化疗患者生前预嘱(advanced directives,ADs)知信行的风险因素并予以分析。方法选取2022年10月—2023年3月于福建医科大学附属协和医院接受化疗的153例老年肿瘤患者作为研究对象。选用ADs知信行调查问卷对患者的ADs知信行现状进行调查。以单因素及多元线性回归分析明确老年肿瘤化疗患者ADs知信行的影响因素。结果老年肿瘤化疗患者ADs知信行评分为(99.25±18.50)分,得分率为76.35%,各维度评分从高到低分别为知识、行为及态度。不同年龄、家庭月收入、退休前职业、居住状况以及有无合并其他慢性病老年肿瘤化疗患者ADs知信行评分差异有统计学意义(P<0.05)。经多元线性回归分析,老年肿瘤化疗患者ADs知信行评分的影响因素包括退休前职业以及家庭月收入(P<0.05)。结论老年肿瘤化疗患者ADs知信行处于中等水平,其影响因素包括家庭月收入及退休前职业,临床工作中可能通过针对上述风险因素进行干预,从而提升该类人群ADs水平。Objective To study and analyze the risk factors of knowledge,attitude and behavior of advanced directives(ADs)in elderly cancer chemotherapy patients.Methods A total of 153 elderly tumor patients who received chemotherapy in Fujian Medical University Union Hospital from October 2022 to March 2023 were selected as the study objects.The knowledge,attitude and behavior of ADs questionnaire was used to investigate the current situation of patients'knowledge,attitude and behavior of ADs.The influence factors of knowledge,attitude and behavior of ADs in elderly cancer chemotherapy patientsy were determined by single factor and multiple linear regression analysis.Results The knowledge,attitude and behavior of ADs scores in elderly cancer chemotherapy patients was(99.25±18.50)points,the scoring rate was 76.35%,and the scores of each dimension were knowledge,behavior and attitude from high to low.There were significant differences in knowledge,attitude and behavior of ADs scores among the elderly cancer chemotherapy patients with different ages,family monthly income,pre-retirement occupation,residence status,and whether they were combined with other chronic diseases(P<0.05).Multiple linear regression analysis confirmed that the factors affecting knowledge,attitude and behavior of ADs score of elderly cancer chemotherapy patients included pre-retirement occupation and family monthly income(P<0.05).Conclusion The knowledge,attitude and behavior of ADs in elderly cancer chemotherapy patients are at a moderate level,and the factors include family monthly income and pre-retirement occupation.In clinical work,it is possible to improve the ADs level of this group through interventions targeting the above risk factors.
