五脏辨治盆腔炎性疾病后遗症的中医思路浅析

摘　　要：五脏各生理功能相互依存、相互制约、相互协调平衡,与人体生理、病理状态关系密切。盆腔炎性疾病后遗症(Sequelae of Pelvic Inflammatory Disease,SPID)以下腹部疼痛、痛连腰骶,伴疲劳、带下增多为主要临床表现。本病发生发展与五脏失调密切相关,脾虚失司、肝郁气结、水火不济、肺失宣降均可致本病。正气不足是SPID发生的内在依据,邪气侵犯是该病发生的重要条件,基于中医学两大基本特点整体观念与辨证论治,文章试从五脏角度探讨SPID的发病机制,根据病因病机选用健脾祛湿、疏肝活血、宣降肺金、交通心肾诸法,并列举辨病与辨证相结合的常用药对,兼顾五脏,使机体免疫力增强,气血流通,阴阳调和,祛除留滞于胞宫之湿浊、瘀血之邪,扶正祛邪,使五脏得安,显著改善患者的局部及全身症状,使SPID得到长期缓解,以期丰富SPID的脏腑辨证理论,拓宽临床诊疗思路。The physiological functions of the five viscera depend on each other,restrict each other,coordinate and balance each other,and are closely related to the physiological and pathological state of the human body.The main clinical manifestations of sequelae of pelvic inflammatory disease(SPID)are abdominal pain,lumbosacral pain,fatigue,and subband increase.The occurrence and development of this disease is closely related to the disorder of five viscera,spleen deficiency,Qi(气)stagnation of the liver,water-fire imbalance,and lung’s failure in descending can all cause this disease.Deficiency of healthy Qi is the internal basis for the occurrence of SPID,and the invasion of pathogenic factors is an important condition for the occurrence of SPID.Based on the two basic characteristics of TCM,namely,holistic concept and syndrome differentiation,this paper tries to discuss the pathogenesis of SPID from the perspective of five viscera.According to the etiology and pathogenesis,several methods including invigorating spleen and removing dampness,soothing liver and promoting blood circulation,promoting lung gold,and communicating heart and kidney are selected,and some common drug combinations combining disease differentiation and syndrome differentiation are listed.Taking into account five viscera,the body’s immunity can be enhanced,and the circulation of Qi and blood and the harmony of Yin(阴)and Yang(阳)can be achieved.The dampness and blood stasis that remain in the uterine cavity can be eliminated,and five viscera can be harmonized,the local and systemic symptoms of patients can be significantly relieved,and the SPID can be alleviated for a long time.This paper enrich the theory of five viscera syndrome differentiation and treatment of SPID,in order to expands the clinical diagnosis and treatment ideas.
