龟鹿强骨汤联合西药治疗老年骨质疏松性腰椎压缩性骨折经皮椎体后凸成形术术后临床研究

摘　　要：目的:观察龟鹿强骨汤联合唑来膦酸注射液及碳酸钙D3片治疗老年骨质疏松性腰椎压缩性骨折(OLVCF)行经皮椎体后凸成形术(PKP)术后患者临床疗效。方法:选取2022年2—12月在安阳市中医院治疗的90例OVCF行PKP术后患者,按随机数字表法分为对照组和观察组各45例。对照组接受唑来膦酸注射液联合碳酸钙D3片治疗,观察组在对照组基础上加用龟鹿强骨汤治疗,2组均治疗6周。比较2组临床疗效、血清骨钙素(BGP)、骨密度(BMD)、骨保护素(OPG)、Oswestry功能障碍指数(ODI)评分、中医证候积分、不良反应和并发症。结果:观察组总有效率93.33%(42/45),高于对照组77.78%(35/45)(P<0.05)。2组OPG、BGP、BMD水平均较治疗前升高,观察组OPG、BGP、BMD水平均高于对照组(P<0.05)。2组中医证候积分、ODI评分均较治疗前降低,观察组中医证候积分、ODI评分均低于对照组(P<0.05)。2组不良反应发生率及并发症发生率比较,差异均无统计学意义(P>0.05)。结论:观察龟鹿强骨汤联合唑来膦酸注射液及碳酸钙D3片治疗OLVCF行PKP术后能改善骨质疏松,改善患者腰椎功能与中医证候。Objective:To observe the clinical effect of Guilu Qianggu Decoction combined with Zoledronic Acid Injection and Calcium Carbonate and Vatamin D3 Tablets on elderly patients with osteoporotic vertebral compression fractures(OLVCF)after percutaneous kyphoplasty(PKP).Methods:A total of 90 cases of OVCF patients who underwent PKP at Anyang Hospital of Chinese Medicine from February to December 2022 were selected and divided into the control group and the observation group according to the random number table method,with 45 cases in each group.The control group was treated with Zoledronic Acid Injection Combined with Calcium Carbonate and Vatamin D3 Tablets,and the observation group was additionally treated with Guilu Qianggu Decoction based on the treatment of the control group.Both groups were treated for six weeks.Clinical effects,serum osteocalcin(BGP),bone mineral density(BMD),osteoprotegerin(OPG),Oswestry Disability Index(ODI)scores,traditional Chinese medicine(TCM)syndrome scores,adverse reactions,and complications were compared between the two groups.Results:The total effective rate was 93.33%(42/45)in the observation group,higher than that of 77.78%(35/45)in the control group(P<0.05).Both groups showed increased levels of OPG,BGP,and BMD compared to before treatment,with the observation group having higher levels than the control group(P<0.05).TCM syndrome scores and ODI scores in the two groups were decreased when compared with those before treatment,with the observation group showing lower scores than the control group(P<0.05).No statistically signifcant differences were observe in the incidence of adverse reation and complications between the two groups(P>0.05).Conclusion:Guilu Qianggu Decoction combined with Zoledronic Acid Injection and Calcium Carbonate and Vatamin D3 Tablets for elderly patients with OVCF after PKP can improve osteoporsis,and improve lumbar function and TCM syndromes.
