基于数据挖掘技术研究耳穴贴压干预过敏性鼻炎选穴规律

摘　　要：目的:基于数据挖掘技术研究耳穴贴压干预过敏性鼻炎(AR)的选穴规律及常用耳穴刺激量。方法:检索中国知网(CNKI)、中国生物医学文献服务系统(CBM)、万方数据知识服务平台(WanFang)、维普网(VIP)、Pubmed、Cochrane Library、Web of Science(WOS)中关于耳穴贴压干预AR的文献,检索时间为建库至2023年8月6日。对纳入的耳穴处方进行频次统计和关联规则分析,并归纳总结常用材料和刺激量。结果:纳入61组耳穴处方,涉及27个耳穴,使用频次排前3的耳穴为内鼻、肺、外鼻。关联规则分析显示以肺-内鼻为最常见的耳穴配伍。耳穴贴压常用材料是王不留行籽,每天按压次数集中在2~4次,单次按压时长集中在1~5 min,耳穴更换频率集中在每天2~3次。结论:耳穴贴压干预AR遵循辨证选穴、病位选穴、神经解剖选穴的原则。耳穴贴压治疗AR最常用的耳穴穴位是内鼻、肺、外鼻、肾上腺、脾、风溪,配穴有咽喉、气管等。Objective:To study the acupoint selection rule and commonly used auricular acupoint stimulation quantities for auricular point pressing intervention in allergic rhinitis(AR)based on data mining techniques.Methods:Literature on auricular point pressing intervention for AR was retrieved from CNKI,CBM,WanFang,VIP,PubMed,Cochrane Library,and Web of Science(WOS)databases,with the search period extending up to August 6,2023.Frequency statistics and association rule analysis were performed on the included auricular acupoint prescriptions,and the commonly used materials and auricular acupoint stimulation quantities were summarized.Results:A total of 61 auricular acupoint prescriptions were included,involving 27 auricular acupoints.The top three most frequently used auricular acupoints were internal nose(TG4),lung(CO14),and external nose(TG1,2i).Association rule analysis showed that the most common auricular acupoint combination was lung-internal nose.The commonly used materials for auricular point pressing was Vaccariae Semen,with the number of daily presses concentrated between two to four times,the duration of each press concentrated between one to five minutes,and the frequency of auricular acupoint replacement concentrated between two to three times per day.Conclusion:Auricular point pressing intervention for AR follows the principles of syndrome differentiation,disease location selection,and neuroanatomical selection.The most commonly used auricular acupoints for treating AR with auricular acupressure are internal nose(TG4),external nose(TG1,2i),lung(CO14),adrenal gland(TG2p),spleen(CO13),and wind stream(SF1,2i),with additional acupoints including pharynx larynx(TG3)and trachea(CO16).
