摘 要:目的:探析在重症肺炎患者行机械通气治疗过程中采取纤维支气管镜与气道廓清联合治疗的临床作用。方法:选取2022年1月~2024年8月本院收治的100例重症肺炎患者(均行机械通气治疗),根据随机抽样法将其分为对照组(常规治疗方案)和治疗组(加用纤维支气管镜和气道廓清联合疗法)各50例,对比两组治疗效果。结果:治疗前两组的各检测指标比较和治疗期间两组出现的不良反应发生率比较,均无统计学意义(P>0.05);治疗1周后,治疗组的动脉氧分压(Partial pressure of oxygen,PaO_(2))、氧合指数(Partial pressure of oxygen/fraction of inspiration O_(2),P/E)比对照组更高,且PaCO_(2)和炎症指标降钙素原(Procalcitonin,PCT)、肿瘤坏死因子-a(Tumor Necrosis Factor-a,TNF-a)、C-反应蛋白(C-reactive protein,CRP)均比对照组更低(P<0.05);治疗1周后,治疗组临床症状缓解更快,患者在ICU的住院时长更短(P<0.05)。结论:对重症肺炎行机械通气治疗患者采取纤维支气管镜与气道廓清的联合疗法能够显著改善患者的血气与炎症水平,缓解其临床症状,缩短住院时间的同时保障患者治疗安全性。Objective:To explore the clinical effect of combining fiberoptic bronchoscopy and airway clearance in the treatment of severe pneumonia patients undergoing mechanical ventilation.Methods:100 severe pneumonia patients(all receiving mechanical ventilation treatment)admitted to our hospital from January 2022 to August 2024 were selected and randomly divided into a control group(conventional treatment plan)and a treatment group(combined therapy with fiberoptic bronchoscopy and airway clearance),with 50 cases in each group.The treatment effects of the two groups were compared.Results:There was no statistically significant difference in the comparison of various detection indicators between the two groups before treatment and the incidence of adverse reactions between the two groups during treatment(P>0.05);After one week of treatment,the PaO_(2)(partial pressure of oxygen)and P/E(oxygenation index)of the treatment group were higher than those of the control group,and the PaCO_(2)and inflammatory indicators PCT(procalcitonin),TNF-a(tumor necrosis factor-a),and CRP(C-reactive protein)were lower than those of the control group(P<0.05);After one week of treatment,the clinical symptoms of the treatment group improved faster and the length of hospital stay in the ICU was shorter(P<0.05).Conclusion:The combination therapy of fiberoptic bronchoscopy and airway clearance can significantly improve the blood gas and inflammation levels of patients undergoing mechanical ventilation for severe pneumonia,alleviate their clinical symptoms,shorten hospitalization time,and ensure the safety of patient treatment.
