摘　　要：目的:分析DWI联合常规MR扫描对肛瘘的诊断价值。方法:选择2020年1月~2023年6月本院收治的17例疑似肛瘘患者,所有患者入院后均接受常规MR检查和DWI联合检查,对比常规MR检查和联合DWI诊断肛瘘价值,包括判定肛瘘单纯性和复杂性,内、外口和肛瘘及脓肿的检查结果,以及对Parks分型的诊断符合率。所有检查结果均与手术结果对比。结果:DWI联合扫描的诊断准确性高于常规MR,与手术对比差异无统计学意义(P>0.05),常规MR诊断内口、瘘管和外口及脓肿的诊断率均明显低于手术结果(P<0.05);手术确诊7例括约肌间型,5例经括约肌型,3例括约肌上型,2例括约肌外型,DWI联合确诊率高于常规MR扫描(P>0.05)。结论:DWI联合常规MR诊断肛瘘可提高肛瘘的检出率。Objective:To analyze the diagnostic value of DWI combined with conventional MR scanning in anal fistula.Methods:A total of 17 patients with suspected anal fistula admitted to our hospital from January 2020 to June 2023 were selected.All patients received conventional MR examination and combined DWI examination after admission.The value of conventional MR examination and combined DWI examination in diagnosing anal fistula was compared,including the determination of the simplicity and complexity of anal fistula,and the examination results of internal,external,anal fistula and abscess.And the diagnostic coincidence rate of Parks classification.All examination results were compared with surgical results.Results:The diagnostic accuracy of DWI combined scan was higher than that of conventional MR,and there was no significant difference between DWI combined scan and operation(P>0.05).The diagnostic rates of conventional MR In the diagnosis of internal stoma,fistula,external stoma and abscess were significantly lower than those of operation(P<0.05).Surgery confirmed 7 cases of intersphincter type,5 cases of transsphincter type,3 cases of suprasphincter type,2 cases of external sphincter type,DWI combined diagnosis rate was higher than that of conventional MR Scan(P>0.05).Conclusion:DWI combined with conventional MR can improve the detection rate of anal fistula.
